PACIFIC MARINE I

A hazmat employer shall ensure that each of its hazmat employees is trained in accordance with the requirements prescribed in this subpart. Hazmat employee training must include the following: (1) General awareness/familiarization training (2) Function-specific training (3) Safety training (4) Security awareness training. Provide evidence of hazmat training for the licensed and unlicensed employees performing hazmat duties on the PACIFIC MARINE 1. 49 CFR 172.702

The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: (i) A self-propelled vessel of 65 feet or more in length, engaged in commercial service. The AIS is inoperable. 33 CFR 164.46(b)

The actions and observations noted in this section shall be entered in the official log book. This section contains no requirements which are not made in other portions of this subchapter, the items being merely grouped together for convenience. (a) Onboard training, musters, and drills: held in accordance with subchapter W (Lifesaving appliances and Arrangements) of this chapter. (c) Drafts and Load Line Markings. Prior to leaving port, ocean, coastwise, and Great Lakes services only. See § 97.15-5. (d) Verification of vessel compliance with applicable stability requirements. After loading and prior to departure and at all other times necessary to assure the safety of the vessel. See § 97.15-7. (e) Loading doors. Where applicable, every closing and any opening when not docked. See § 97.15-17. (f) Hatches and other openings. All openings and closings, or leaving port without closing. Except vessels on protected waters. See § 97.15-20. (g) Emergency Lighting and Power Systems. Weekly and semi-annually. See § 97.15-30. The entries required above were not found in the logbook for the vessel. 46 CFR 97.35-5

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.60; Radio lifesaving appliances. Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. The HF radio is not connected to primary power. 46 CFR 90.27-1

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. IAW 46 CFR 112.05-5(a); The emergency lighting requirements of § 112.15-1 (b), (c), (f), and (g) must be met. Requirements of Subpart 112.39 must be met by the relay-controlled, battery-operated lanterns. Emergency power supply must be at least 6 hours. The stbd side emergency light in the lazarette was found inoperable. 46 CFR 90.25-1

Shutoff valves must be installed in the fuel supply lines, one as close to each tank as practicable, and one as close to each fuel pump as practicable. Valves must be accessible at all times. The port side fuel tank remote fuel shutoff valve was inoperable when tested. 46 CFR 56.50-75(B)(4)

Each pump must be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 PSI. Fire pump was unable to prime and produce required pressure to the fire main system. 46 CFR 95.10-5(c)

Each machinery space must be ventilated to ensure that, when machinery or boilers are operating at full power in all-weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery and for the safety, efficiency, and comfort of the crew. Stbd engine room dampener is seized and unable to fully cycle open or close. 46 CFR 58.01-45.

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. The stbd ring buoy life light was found with conductors soldered instead of the screw down connection. 46 CFR § 90.27-1

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. The flexible fill piping on the fuel tanks is not bonded from the metal hull to metal tank. 46 CFR § 90.25-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. The vessel markings were found faded on the ring buoys. 46 CFR § 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Three flares were found onboard. The vessel is required to have a minimum of 12 onboard. 46 CFR § 90.27-1

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. The bilge overboard skin valve was found installed after a flexible hose at the side shell. 46 CFR 56.50(c)(3)(iv) prohibits this installation. 46 CFR § 90.20-1

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date, except as required in paragraph (b) of this section. The vessel opeator missed two annual vessel inspections. The COI is no longer valid and the vessel is not permitted to carry freight for hire. 46 CFR § 91.27-1

Remote shutoff valves at the operating station are not labeled for immediate identification. 46 CFR 56.50-75(B)(4)

Engine room dampeners are seized and unable to fully cycle open or close. 46 CFR 58.01-45.

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder angle indicator at the main steering station in the pilothouse is not functional. 46 CFR 58.25-25(a).

Electric installations on vessels must ensure: Maintenance of services necessary for safety under normal and emergency conditions. The starboard battery voltage meter is not functional at the operators station. 46 CFR 111.01-1(a)(1).

Automatic bilge pumps must - Be provided with bilge high level alarms that annunciate in the machinery spaces and at a manned control location and are independent of the pump controls. Engine room high level visual alarm was not operational at the operating station. Audible alarm was functional. 46 CFR 62.35-10(a)(1)

(1) Drills must, as far as practicable, be conducted as if there were an actual emergency. (2) Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. The crew members were participating in drills once every three months instead of once a month. Provide evidence of monthly drills IAW 46 CFR 199.180. 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. As per 46 CFR 199.180(h); When musters are held, details of abandon-ship drills, fire drills, drills of other lifesaving appliances, and onboard training must be recorded in the vessel's official logbook. Logbook entries must include— (i) The date and time of the drill, muster, or training session; (ii) The survival craft and fire-extinguishing equipment used in the drills; (iii) Identification of inoperative or malfunctioning equipment and the corrective action taken; (iv) Identification of crewmembers participating in drills or training sessions; and (v) The subject of the onboard training session. Provide logbook entries for all drills conducted with the crew. 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Training material must be on board each vessel and must consist of a manual of one or more volumes written in easily understood terms and illustrated wherever possible, or of audiovisual training aids, or of both as follows: (2) The training material must explain in detail— (i) The procedure for donning lifejackets, immersion suits, and anti-exposure suits carried on board; (ii) The procedure for mustering at the assigned stations; (iii) The procedure for boarding, launching, and clearing the survival craft and rescue boats; (v) The procedure for releasing survival craft from launching appliances; (vii) The illumination in the launching areas; (viii) The use of all survival equipment; (x) With the assistance of illustrations, the use of radio lifesaving appliances; (xiv) The hazards of exposure and the need for warm clothing; (xvii) All other functions contained in the muster list and emergency instructions; and (1) Every crewmember with emergency duties assigned on the muster list must be familiar with their assigned duties before the voyage begins. Provide emergency procedures for all required equipment onboard as per 49 CFR 199.180(a). 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, as applicable, a demonstration of— The proper servicing of each inflatable liferaft and inflatable lifejacket has been serviced as required under this chapter; The ability of crew to effectively carry out abandon-ship and fire-fighting procedures. The life raft was not onboard due to servicing. Provide the annual servicing certificate and install the life raft for the inspection. MI shall witness the crew perform abandon-ship and fire-fighting procedures IAW 46 CFR 199.45(b). 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.60; Radio lifesaving appliances. All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.70(b)(2)(iii); The lifejackets stowage positions must be marked with the words “ LIFEJACKETS” or “CHILD LIFEJACKETS” as appropriate, or with the appropriate symbol from IMO Resolution A.760(18). Each lifejacket, immersion suit, and anti-exposure suit container must be marked in block capital letters and numbers with the quantity, identity, and size of the equipment stowed inside the container. The life jacket stowage containers located fwd of the operating station are not marked accordingly. 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.110(c); Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. The life raft embarkation station is not adequately illuminated with emergency power. One dim light was found installed facing the life raft. 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.80(a); Clear instructions must be provided on the vessel that detail the actions each person on board should follow in the event of an emergency. There are no posted emergency procedures for crew members onboard. 46 CFR 90.27-1

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. IAW 46 CFR 199.60(a); Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. The FCC station authorization and FCC Safety Certificate are expired. 46 CFR 90.27-1

Each general alarm contact maker must be marked in accordance with requirements in subchapter J (Electrical Engineering Regulations) of this chapter. There is no general alarm contact maker onboard. 46 CFR 97.37-5

All self-propelled vessels shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans:General arrangement plans showing for each deck the fire control stations, the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. A general arrangement plan was not found onboard. 46 CFR 97.36-1

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. Provide enough lights throughout machinery spaces to allow essential operations and observations under emergency conditions and to allow restoration of service. Provide at least one working light in the steering gear room. 46 CFR 90.25-1

Each vessel must undergo drydock and internal structural exams as per Table 91.40-3(a). The PACIFIC MARINE I was due for a dry dock and internal structural exam on March 31, 2024. The vessel is not permitted to operate until a dry dock and internal structural exam is completed. 46 CFR 91.40-3

Each life buoy must be marked with VSL name. Marked VSL name on the new life buoys.

VSL must have Marine Charts of the operation area. Current charts are water damaged. Replace damaged marine charts.

Life ring must have lifeline, non kinking, not less than 5/16" diameter. Replace deteriorated lifeline.

Replace two portable CO2 with deteriorated exterior.

Replace inoperable temperature gauge for STBD propulsion engine.

VSL sustained hull damage (12" weld crack)on top of the side wall of the cargo deck area, 2 FT from the engine room FWD BULKHD.

Each life buoy must be marked with VSL name. Marked VSL name on the new life buoys.

Each life jacket must have have approved light attached to the front. Install missing lights and repair inoperable lights.

Electrical installations must ensure maintenance of service necessary for safety under normal and emergency conditions. Repair ship generator and demonstrate proper operations.

Repair inoperable RPM gauge for STBD propulsion engine.

Remove oil absorbent pads from engine room bilge.

Bilge pipe manifold must be labeled for each compartment.

Provide current Notice to Mariner publication.

Replace inoperable pressure gauge on fire pump.

Replace deteriorated hydraulic hose on head tank for steering hydraulic system.

Unusual non - metallic noise from steering gear pump.

Re-label missing breaker names on breaker panel in the wheel house.

Attach missing life jacket lights on those life jackets identified without lights.

Replace two ring life buoys with damaged outer protection cover.

Missing 12 hand rocket parachute flares.

Crew unable to demonstrate shut down on propulsion engines and generator.

Missing warning sign for ventilation blower breaker shut down for engine room.

Medical certification for master is expired.

Air filters for port & starboard propulsion engines are clogged.

Main propulsion engines must be suitable for intended service. Starboard engine sustained damage to cylinder head gasket. Conduct repairs I.A.W. engine manufacture procedure.

Starboard pulley on cargo ramp failed due to corroded fastener. Replace damage fastener with in-kind fastener and show proper ramp operation.

Replace damaged valve (FV-5) after engine room.

Remove all flammables (paint, trash, carboards) from the engine room.

Connect electrical wire to engine room light fixture.

Install missing gauges for generator and propulsion engines in the engine room.

Provide proof of annual service performed for all fire fighting extinguishers.

Replace (03) damaged ring life buoys.

Repair air bound fire pump and demonstrate proper operations.

Replace damaged gaskets for cargo deck manhole covers.

Main propulsion shaft must be equivalent to standard established by ABS. VSL lost port shaft due to shaft failure (Broken). Replace port shaft with in-kind shaft and be prepared to conduct operational test to be attested by the attending MI.

VSL must meet radio installation in 47CFR80. Onboard SSB is nonoperational. Repair SSB and show proper operation to attending MI.

At least have of life buoy must have CG approved self-igniting light as required by 46CFR199.70. Replace non CG approve light on life buoy.

VSL must be equipped with independent driven fire pump as required by 46CFR95.10-5. Repair non operational fire pump and show proper pump operations.

Repair damaged and holes flexible non metallic hose on port fuel tank.

Install missing cutter pin on port hydraulic ram connecting pin.

Each life raft must be service annual as required by 46CFR199.190. Provide current proof of annual service conducted on life raft.

Replace damaged hydraulic hose on hydraulic oil reservoir in STBD engine room.

Repair wasted and holed exhaust muffler on port and STBD engines.

Replace wasted bulkhead penetration fitting on aft engine room bulkhead.

Stern light and anchor lights are inoperable. Conduct repairs and show proper light operations to MI.

Missing marine charts. Provide up-to- date marine charts.

Vessel requieres to have 6 red & 6 orange flares or 12 combinations as requried by 46 CFR 117.68. Found 8 day & night flares on board. Provide additional flares.

Ring life buoy must have a life line as required by 46 CFR 117.70(c). Install a 60', buoyant, non-kinking, dark color, life line to ring life buoy.

Battery must be maintain in optimal condition for its service as required by 46 CFR 111.01-1. Replace low voltage battery for generator.

receptacle outlet must have plug opening uncovered as required by 46 CFR 111.79. Install missing outlet cover engine room forward bulkhead.

Engine room space must made of A-60 structure. Engine room over head where wires and tubes penetrate bulkhead deck to wheel house must meet A-60 requirements.

Vessel must have operable propulsion engine RPM gauges. Show proper operation of the RPM gauges in wheel house and engine room.

Found 9" crack on main deck, port side of wheel house. Repair crack IAW guidance on provided NVIC 7-68.

Found inoperable emergency shutdown for port propulsion engine. Conduct repairs and show proper engine shut down operation.

Wasted and holed salt water pipe to port engine exhaust muffler. Repair wasted pipe.

Missing hotline number for EAP display.

Provide proof training for new crewmembers before assuming safety sensitive position.

Provide proof of 1 hour EAP training for supervisors/DER.

Missing SAP name and contact number

Vessel sailed short in October 2016 trip from Saipan to Guam (short one deckhand). Duration of voyage was 16 hours.

No proof of DAPI enrollment or negative drug test of vessel crew.

Vessel Captain could not provide FCC Radio Opertors license to USCG.

Audible high water bilge alarm inopertive at operators station.

No installed fixed bilge pump or manifold in aft steering compartment (lazarettte).

Hydrostatic release unit was improperly installed. Vessel rep is to also provide expiration date of hydro-release unit.

Vessel missing second required lifebuoy light as required by COI.

Remove all oil and oily debris from lazarette/steering gear compartment.

Aft machinery space bulkhead observed holed in various locations. Bulkhead is required to be watertight.

Charts for operating area (Saipan & Guam) were outdated and not corrected. Vessel Captain was not familiar with using CG BNM's or chart corrections.

Tide Tables were also missing onboard.

Captain was not able to provide records of completion for required emergency drills, trainging and musters (Sub W).

Anchor shackles wasted and threads of pin rusted. Crew unable to unscrew pin.

Dead-end wiring under operators console (behind starboard hatch cover).

Broken deck railing on starboard quarter.

No records of shipboard garbage disposal.

PFD's observed padlocked in storage containers. Contractor equipment was also obstructing storage containers.

Vibration Dampner on Port tailshaft failed.

Explain purpose of micro-switches installed on both main propulsion engines.

Starboard navigation light cover is bent and obstructing arc of visibility of light.

Amber stern towing light inoperative.

Cargo on deck appears to violate conditions of COI for carrying cargo. Vessel rep to provide information to CG Marine Inpector on how current cargo configuration meets allowable Stability parimeters.

Soft on fire main piping. Also observed leaking.

Soft patches on bilge piping in machinery space, in way of starboard main propulsion engine.

Vessel shall have operational bell as required by NAV Rules. Repair and demonstrate proper bell operation.

All hand fire extinguisher shall be service annual as required by NFPA. Found service tag on 5-portable CO2s with last service 01 Mar 2015.

Engine room shall have emergency light system as required by 46 CFR 112.43. Found inoperable emergency light in engine room. Repair and demonstrate proper emergency light operation.

Found light fixture in steering gear compartment missing light bulb and protective cover. Repair and demonstrate proper light operation as required by 46 CFR 111.75

Vessel must have first aid kit with up-to-date contents as required by 46 CFR108.707. replace all identified items with expired dates.

Lifejacket shall be marked with the name of vessel as required by 46 CFR 91.25-15. Marked all child lifejackets with vessel name.

Found dried and deteriorated grab lines on 3- lifebuoys. Replace grab lines with in-kind material. 46 CFR 199.211

Hydraulic hose shall not be subjected to twisting as required by 46 CFR 58.30-20. Re- install steering hydraulic hose on the support brackets and away from the bilge.

Vessel must have proper day shapes for towing evolution as required by NAV RULES. Provide proper day shapes for towing on board.

Vessel operates within Sea Area A1 must upgrade to VHF-DSC radio as required by 47CFR80.905 & MSIB 15-15.
