The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel suffered a major marine casualty while in port to crane #1 on the cargo deck. PSCO attended vessel and confirmed damage to vessel structure, cargo, and subsequent crane equipment. 40AC

PSC team conducted In-Service exam after vessel received damage from storm. PSCO discovered 13 holes on cargo holds #1 and #2, damaged handrails on port and starboard sides of vessel forward of cargo hold #3, damaged ballast tank vent near cargo hold #2 on starboard side. Provide documentation from certificate issuing authority attesting to repairs. -74 SOLAS(14) I/11a
