The OCMI requires the master to have sufficient visibility of the recovery station and for a rescue ladder to be used. The vessel's master did not have sufficient visibility and no ladder was used.

The bilge alarm panel continuously sounds because of a faulty alarm in the lazarette. The visual alarms also failed to activate in the bow thruster room and the engine room.
