The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Hull and internal structure was damaged during a marine casualty which caused a breach in the hull and subsequent flooding in the starboard forward void and the storage space inboard of the void. Permanent repairs will need to be completed prior to departing repair facility in Cincinnati, OH.
Action required: 15 - Rectify deficiencies by next port

Vessel was reported to be operating on the Ohio River in the vicinity of mile marker 447 without a valid COFR. Vessels voyage was terminated and the vessel was moored to the C & B Marine, Hebron docks in Kentucky until all paperwork is submitted by the company demonstrating oil spill liability insurance and verbal compliance is received from the NPFC.

Company must obtain an approval letter or IOA letter prior to 16JUN2014
