The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a
design and construction adequate for the service for which they are intended and shall be so installed and
protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Marine Inspector identified the Quick Closing Valve leaking fuel oil from to locations on the valve on #1 Ships Service Generator. Crew tried adjusting the stem packing with no success. **Generator will not be operated until valve has been repaired or replaced.

The vessel operator of each vessel required by § 155.720 to have transfer procedures shall maintain them current and shall require vessel personnel to use the transfer procedures for each transfer operation. The vessel's transfer procedures require a preventer pin to be installed top dead center of each flange bolt circle, designed to further identify these flanges as vapor recovery connections and not for the transfer of liquid cargo or ballast. Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between bolt holes, and in line with the bolt hole pattern. The port and starboard vapor recovery piping located at the manifold does not have a stud installed top dead center on the fixed piping flange face. The Port and Starboard aft Vapor Emission Control System manifolds did not have the Permanent studs installed. **Rectify prior to loading.

Resolution: ABS: Recommended: Install the missing pin to both port side after vapor recovery flanges. To rectify this requirement a hot work permit is required. The vessel was offloading cargo which impede from completing the repairs at the time of the survey.
Your vsl is required to have 03 oilers and only has 02 onboard. You are permitted to make a one-time voyage to Beaumont, TX with one less oiler than required by your COI. You must fill the position prior to departing Beaumont, TX with a properly qualified individual. 46 CFR 15.515(a)

Vessel is not properly manned IAW with COI.Vessel is to properly man the vessel IAW Certificate of Inspection and provide updated crew list to local OCMI, prior to continuing its voyage.
