Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500 L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO observed quick closing valve for LSMGO service tank, which was currently in use, intentionally blocked open with sockets which prevented the valve from being closed from outside the space during an emergency and was unable to confirm proper operation of the valve. 74SOLAS (2009 Cons.) The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. As noted in deficiency 01, PSCO observed the quick closing valve for the LSMGO service tank blocked open. The [name withheld] stated that the valve had been blocked open a week earlier during routine testing to prevent inadvertent shut down of running machinery. Chief Engineer stated he was unaware of valve blockage. The vessel was unable to provide any procedures for testing quick closing valves or safety guidance prohibiting quick closing valve blocks. 74 SOLAS (2020 cons.) IX/3.1, ISM 10.3
