The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguishers are inspected and maintained in accordance with NFPA 10. During the inspection of the cargo pump prime mover space one fire extinguisher gauge was discovered in the red. Satisfactory evidence could not be provided of adequate monthly servicing. Replace/renew fire extinguisher identified during inspection and provide sufficient documentation (photo/email statement)once it is back on station and ready for immediate use.46 CFR 31.10-18(a) NFPA 10

Vessel representatives presented hose test records attesting to insufficient test psi. (150spi) for both port and starboard cargo hoses and the vapor hose. Prior to transfer operations ensure all cargo hoses are tested to the required psi (1.5x MAWP) Test target based on service pressure of 200psi is 300psi. Provide documentation attesting to sufficient hose test.

Vessel observed using an electric blower fan through an open cargo pump pressure gauge porthole to cool the cargo pump engine room compromising the gastight bulkhead. Cargo pump engine room was uncomfortably warm despite exterior freezing temperatures. Engine can reach 200 deg F during normal operations. Install sufficient ventilation in compliance with the vessel's approved hazardous zone plan.

Resolution: Porthole closed with nuts and bolts and vessel rep instructed to keep the porthole closed unless the barge is certified gas free. Hinged porthole must be replaced with a fixed window type by next credit drydock. Note added to vessel details.
