No less than two of these (lifebuoys) shall be capable of quick release from the bridge. Quick release pin unserviceable and twisted around the frame.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The objective evidence of the deficiencies below brings into question the validity of the safety management system as required in the ISM Code. An audit is required to be submitted to the Coast Guard prior to depart. The vessel's SMS states: Vessel's safety and operations manual; shipboard operations - engine room machinery, engineer duties and knowledge to include: oil record book maintained IAW MARPOL Regulations, Fire precaution in machinery spaces to provide proper cleaning, especially oil spills and leaks. Cargo operations; load cargo IAW specific approved manuals, e.g., stability manual, cargo securing manual. During an examination of the Oil Record Book (ORB), PSCO found that incorrect codes have been used for the last three years of ORB logbook entries. During the engine room examination, PSCO observed insufficient cleanliness of engin room with excess amount of oily mixture in bilges. During the deck walk examination, PSCO found that both starboard and port load lines were fully submerged. 74 SOLAS 01 IX/3.1 ISM CODE PART A/10.1

The appropriate load lines on the sides of the ship shall not be submerged at any time when the ship puts to sea. The port and starboard load lines were found fully submerged. On the port side, the entire plimsol mark is submerged, and the starboard load line is submerged two inches. ICLL 66 Art. 12(1)

The purpose of this chapter is to require the fullest practicable degree of fire protection. PSCO observed insufficient cleanliness of engine room with excess amount of oily water mixture in bilges, creating an excessive fire hazard.

The Oil Record Book, whether as a part of the ship’s official log book or otherwise, shall be in the form specified in Appendix III to this Annex. Disposal of bilge water which has accumulated in machinery spaces shall be recorded as a code (letter) (D) with corresponding item (number), vessel’s CE is recording the method of discharge, transfer, or disposal of bilge water as a code C item 11 and 12.

Fire extinguishers shall be periodically examined and subjected to such tests as the Administration may require. Fire extinguisher #11 (per the fire control plan) within the engine room was found with no charge.

1110 - Cargo Operations - All cargoes other than solid and liquid bulk cargoes , cargo units and cargo tranport units, shall be loaded, stowed, and secured throughout the voyage in accordance with the cargo securing manual approved by the Administration. Cargo securing manual requires that container twistlocks be welded to the deck. Twistlocks are not welded and are wasted and rusted preventing them from being locked. - 74SOLAS(14 Cons.) 2490 - For each ship subject to regulations 5 and 6 of this Annex, details of fuel oil for combustion purposes delivered to and used onboard shall be recorded by means of bunker delivery notes that shall contain at least the information specified in appendix V to this Annex. Chief Engineer could not provide PSCO's with Sulphur content of recent fuel deliveries. - MARPOL(17 Cons.) Annex VI/18.3.5 - 10c

1030 - Steering Gear Alarm - In the event of a power failure to any one of the steering gear power units, an audible alarm and visual alarm shall be given on the navigation bridge. Audible alarm on bridge is inoperable. - 74SOLAS(04 Cons.) II-I/29.5.2 - 17ac

1030 - Sterring Gear Alarm - A low level alarm for each hydraulic fluid resevior to give the earliest practicable indication of hydraulic fluid leakage. Audible and visual alarms shall be given on the navigation bridge. There are no low-level alarms on the navigation bridge. - 74SOLAS(04 Cons.) II-I/29.12.2 - 17ac

0735 - Personal Equipment - Four sets of full protective clothing resistant to chemical attack shall be provided in addition to the firemans's outfits required by regulation 17. Vessel is authorized to carry dangerous goods. Vessel crew could not provide the four chemical suites that are required to be kept onboard. - 74SOLAS(01 Cons.) II/2/54.2.6.1 - 16ac

Self-activating smoke signals shall be provided with a quick-release arrangement that will automatically release and activate the signal and associated self-igniting light connected to a lifebuoy having a mass of 4kg. Both quick release lifebuoys are unable to self-activate smoke signal.- 74 SOLAS (14 Cons.) III/34 - 17 LSA II/2.1.3.6

Every lifebuoy intended for quick-release shall have a mass not less than 4kg. Starboard quick-release lifebuoy has a mass less than 4kg. - 74 SOLAS (14 Cons.)III/34 - LSA II/2.1.1.7

Oil level gauges with flat glasses and self-closing valves between gauges shall be maintained in the proper condition to ensure accurate function. Self-closing valves on dirty oil, day tank and fuel oil tank were permanently fixed in open position. - 74 SOLAS (2001 Cons.) II-2/15.2.6.2.2

SOLAS 14(cons) III/13.4.2 - Each liferaft shall be stowed with a float-free arrangement so that each floats free. Port and Starbaord liferafts had sea painter lines which were tied to the ship instead of the weak link. Both arrangements were corrected on the spot.

SOLAS 14(cons) V/28.1 - All ships shall keep on board a record of navigation activities which much contain sufficient detail to restore a complete record of the voyage. The bridge log book contained inaccurate time/date of pilot onboard and lacked destination of voyage.

The condition of the ship and its equipmemt shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respect will remain fit to proceed to sea without danger to the ship or persons on board. Vessel lost main steering during manuvering drills while enroute to Port Everglades. The steering loss was due to a faulty breaker. 74 SOLAS (2014) 1/11a

1099: Bilge wells shall be monitored in such a way that accumulation of liquids is detected. Bilge alarms located in engine room and steering gear room had rags around float preventing actuation and removed from bilge well in steering gear room. 74 SOLAS (04 Cons) II-2 Reg 48.1

0750: Doors required to be self-closing shall not be fitted with hold back hooks. Door to engine room and escape trunk door were held open with hold back hooks and stairway trunk door was not self-closing. 74 SOLAS (01 Cons) Ch II-2/Reg 47.2

1270: All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel fitted so that it is weather-tight when closed. Second deck starboard door lower coaming is wasted/holed and not weather-tight. ILLC 12.1

1720: A ship of 300 GT or more must have a fixed container under each fuel oil tank vent, overflow, and fill pipe that has a capacity of at least one-half barrel. Fuel oil fill pipes and tank vents do not have adequate containment capacity. 33 CFR 155.320
