The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO received report of fuel oil leakage into the cargo hold no.6 starboard side due to damage caused by a corroded sounding pipe. 74 SOLAS (20) II-2/4.1.1

The master of a vessel equipped with ballast tanks that operated in the waters of the United States must employ one of the following ballast water management methods: Install and operate a ballast water managament system (BWMS) that has been approved by the Coast Guard. Following installation, the master of the vessel must properly maintain the BWMS in accordance with all manufacturers specifications. Vessels installed BWMS was not operational. 33 CFR 151.2025A(1)

In a ship which oil fuel is used, the arrangements for the storage, distribution, and utilization of the fuel oil shall be such as to ensure the safety of the ship and the persons on board. PSCO observed the fuel oil quick closing valve was braced open and preventing proper function. 74 SOLAS (20)II-2/4.2.2

Resolved 4 October 2021
Resolution: Chief Engineer removed obstruction that was holding the valve open.
