THE COMPANY AND SHIP SHALL COMPLY WITH THE REQUIREMENTS OF THE INTERNATIONA SAFETY MANAGEMENT CODE. FOR THE PURPOSE OF THIS REGULATION, THE REQUIREMENTS OF THE CODE SHALL BE TREATED AS MANDATORY. PSCO OBSERVED HEAT DETECTION TEST IN PURIFIER ROOM WITH AN OPEN FLAME. SMS STATED TESTING MUST UTILIZE A HEAT FUN. MASTER TO PROVIDE TRAINING TO CREW ON UTILIZATION OF SMS.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purposethe following functional requirements shall be met: means shall be provided to limit the accumulation of flammable vapours and the ignitability of combustible materials should be restricted. PSCO found two (02) 45-gallon trash bags filled with oily rags. Additionally oil and oil soaked rags were found sporadically throughout the engine room indicating leaks. 74 SOLAS (2009) II-2/4

The condition of the ship and its equipment shall be maintained to conform with the provision if the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO found missing lagging throughout the engine room as well as improper pipe insulation and utilizing aluminum foil and duct tape. This was especially apparent forward of the main engine. 74 SOLAS (2009) I/11

Oil filtering equipment shall be of a design approved by the Administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. In considering the design of such equipment, the Administration shall have regard to the specification recommended by the Organization. While testing oil content meter the crew could not demonstrate a continuous reading of 18 months as per the systems type approval. Additionally the crew could not provide the manufacturers manual. Each incinerator on a ship constructed on or after 01 July 2000 shall meet the requirements contained in Appendix VI to this Annex. Each incinerator subject to this paragraph shall be approved by the Administration taking into account the standard specifications for sipboard incinerators developed by the Organization. Muster and embarkation stations shall be adequately illuminated by lighting supplied from emergency source of electrical power. While testing emergency lighting at port side life raft embarkation, the emergency lighting did not work. PSCO found damaged and burnt electrical plug. 74 SOLAS (2014) III/11.4
