Discharge of cargo residues that cannot be recovered using commonly available methods for unloading, where all the following conditions are satisfied: cargo residues, cleaning agents or additives, contained in hold washing water do not include any substances classified as harmful to the marine environment, taking into account guidelines developed by the Organization. Sector NOLA arrivals received report of a fire in vessels cargo hold. Upon exam of the cargo hold by PSCO, it was noted that the majority of the cargo hold to include cargo hatch interior surfaces are covered in byproduct of the cargo that was burnt leaving a visible residue that the vessels master stated would be cleaned by a commercial contractor prior to departing the vessels current location. Vessel is required to submit documentation to this office, regarding the cleaning and disposal of contaminants from cargo hold fire. Specifically, the method in which the cargo hold will be cleaned and where removed containments will be landed.

FOLLOWING THE FIRE IN CARGO HOLD #2 THAT OCCURED ON 7 DEC 21 THE VESSEL REPORTED THE ACCIDENT TO THE COMPANY. THE VESSEL AND COMPANY HAVE NOT PROVIDED TO THE USCG OBJECTIVE EVIDENCE THAT A POST ACCIDENT INVESTIGATION AND ANALYSIS HAS BEEN COMPLETED. THE OBJECTIVE OF THE INVESTIGATION SHOULD FOCUS ON CAUSAL FACTORS AND PROVIDE CORRECTIVE ACTIONS TO IMPROVE VESSEL SAFETY. AN INTERNAL AUDIT WITH A FOCUS ON THE AFOREMENTIONED INCIDENT IS REQUIRED WITHIN 90 DAYS.

1400 The condition of the ship and its equipment shall be maintained to conform with the provisions of the rpesent regulations to ensure the ship will remain fit to proceed to sea without danger to the ship or persons on board. Vsl experienced an ICU #2 failure while transiting the navigable waters of the US which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel. Submit for review a Class report attesting to the cause of the equipment failure, that authroized temp or permanent repairs have been completed, and the vsls full and normal maneuverability has been restored. 74SOLAS(14) I/11 10c.
