FOR THIS PURPOSE, THE FOLLOWING FUNCTIONAL REQUIREMENTS SHALL BE MET: MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED FUEL OIL ACCUMULATED ON THE #2 AND THE #5 FUEL PUMP FOR THE MAIN DIESEL ENGINE. PSCO ALSO OBSERVED ACCUMULATED OIL BENEATH GRATING BETWEEN THE #2 AND #3 GENERATORS. 74 SOLAS (20) CHAPTER II-2 REG 4.1.1 / 17ac.

OBJECTIVE EVIDENCE DURING AN EXPANDED ISM EXAM REVEALED THAT THE CREW WAS NOT ACCURATELY FOLLOWING THE SHIP’S PROCEDURES WITH REGARD TO THE INSPECTION OF THE C02 BOTTLE STORAGE ROOM. IT IS RECOMMENDED THAT AN EXTERNAL THIRD PARTY AUDIT OF THE SMS BE CONDUCTED WITH REGARD TO DEFICIENCY NO. 1. 74 SOLAS (14) IX/ 3.1, ISM CODE PART A 10.1- 30ac AFTER ISSUANCE OF DEFICIENCY CLASS ATTENDED VESSEL AND REQUIRED AN ISM AUDIT WITHIN 90 DAYS. DEFICIENCY WAS CHANGED FROM AN ACTION CODE OF 30 TO 40 TO ENSURE THE PROPER ACTIONS ARE TAKEN PRIOR TO THE VESSELS RETURN TO THE U.S.
