Guard rails fitted on superstructure and free board decks shall have at least three courses. The opening below the lowest course of the guard rails shall not exceed 230 mm. The PSCO observed that the starboard bows guard rails lowest course was damaged due to rough seas and that the damage exceeded 230 mm.

Two holes have been drilled through the bulkhead on B deck from the 'Pipe & Elect Cable & Duct Space' into the athwartship corridor. In addition, fire break was removed from the cable penetration box on D deck and replace with unknown sealant from the same space. In both cases, cable was run through the penetrations for lighting and shipboard internet. No approval documentation was available for these penetrations. 74 SOLAS (2014 Cons) II-2/9.3

Resolution: The holes in the bulkhead on B deck was welded. The cabless of lighting and shipboard internet were installed through the existing cable penetrations. The unknown sealant on the deck D was replaced with an approved one, approval certificate on board.
