A GAS CARRIER SHALL COMPLY WITH THE REQUIREMENTS OF THE INTERNATIONAL GAS CARRIER CODE. GAS DETECTION EQUIPMENT SHOULD BE SO DESIGNED THAT IT MAY BE READILY TESTED. PSCO OBSERVED A LEAK IN THE FIXED GAS CABINET FROM ONE OF THE CARGO HOLD SPACES. 74 SOLAS (24) VII/13.1 IGC 93 13.6.6

THE RING, LINES, AND LETTERS SHALL BE PAINTED IN WHITE OR YELLOW ON A DARK GROUND OR IN BLACK ON A LIGHT GROUND. THE MARKS SHALL BE PLAINLY VISIBLE AND, IF NECESSARY, SPECIAL ARRANGEMENTS SHALL BE MADE FOR THIS PURPOSE. PSCO OBSERVED THAT THE LOAD LINE MARK WAS NOT OF WHITE OR YELLOW ON A DARK BACKGROUND. (f) Each flammable gas detection system must have an audible and visual alarm for power failure and loss of gas sampling flow; and (s) Each flammable gas detection system must have at least one flow meter capable of measuring the flow to the gas analyzer, and must provide a means for ensuring that there is a positive flow in the right direction in each sampling line at all times. During a test of the fixed gas system, it was found that the low flow alarm was not being activated and the flow meter was not responding to flow changes. The flow was tested from the number 4 tank holdspace aft and locally in the cabinet. The crew disassembled and repaired the flow meter. After further troubleshooting, the crew could not get the low flow alarm to work properly or in accordance to design. 46 CFR 154.1350(f) & (s) 17ac

0735 - All of the 06 required fire fighter’s outfits onboard were in significant degraded condition as evident by excessive amounts of torn and missing fire protective shielding; additionally, 01 self contained breathing apparatus regulating valve was stuck in the open position and is in an inoperable condition rendering vessel unfit for fire fighting readiness. 74 SOLAS (14) II-2/14.2.1.2 30abc
