Engine exhaust cooling system built in accordance with the requirements of ABYC P-1 (incorporated by reference; see 46 CFR 175.600) will be considered as meeting the requirements of this section. ABYC P-1: 1.7.10.1, Every exhaust hose connection shall be secured with at least two clamps at each end to produce a secure, liquid and vapor tight joint. Marine Inspector identified both main engines missing additional second clamps on engine exhaust cooling system near temp sensor. STBD engine exhaust elbow was also seeping raw water at the second clamp. Repair and Replace to the satisfaction of CFR and Marine Inspector. 46 § 182.425 (c) Engine exhaust cooling.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Marine Inspector identified significant gaps on both supply and exhaust openings during the manual ventilation closure exercise. Repair both closure devices to satisfactorily seal openings to the satisfaction of the Marine Inspector. 46 § 182.465 (h) Ventilation of spaces containing diesel machinery.

A hatch cover must have securing devices. Marine Inspector identified both weather deck forward and aft hatches unable to secure to create a watertight boundary. Repair or replace hatches to provide watertight integrity to the satisfaction of the Marine inspector. 46 § 179.360 (b) (1) Watertight integrity

Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Marine Inspector identified both aft weather deck hatches to be missing captive chains. Replace to the satisfaction of the Marine inspector. 46 § 179.360 (b) (2) Watertight integrity

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Marine Inspector identified several dead-end wires in the engine room above aft generator, in the forward portion of engine room near the chiller, and at the control station near the helm. 05 receptacles were also missing covers near the bar in the accommodation space. Dead-end wires shall be secured and taped, and receptacle covers shall be replaced to the satisfaction of the Marine Inspector. 46 § 183.200 (b) General design, installation, and maintenance requirements.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Marine Inspector identified engine room lighting missing lens. Replace lens or replace with LED bulbs that provide built in lens to meet the requirement of the CFR and to the satisfaction of the Marine Inspector. 46 § 183.410 (a) Lighting Fixtures

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Fuel valves were not marked in the direction of operation. Install placard to the satisfaction of the Marine Inspector to meet CFR requirements. 46 § 185.608 Fuel shutoff valves

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Marine Inspector identified port battery box in engine room with an unsecured lid. Secure all battery box lids to the satisfaction of the CFR and Marine Inspector. 46 § 183.350 (b) Batteries—general.

Fill pipes and sounding pipes must run as directly as possible, preferably in a straight line, from the deck connection to the top of the tank. Such pipes must terminate on the weather deck and must be fitted with shutoff valves, watertight deck plates, or screw caps, suitably marked for identification. Gasoline fill pipes and sounding pipes must extend to within one-half of their diameter from the bottom of the tank. Diesel fill pipes and sounding pipes may terminate at the top of the tank. Marine Inspector identified fuel fill was not marked for identification of fuel type. Label fuel fill with “Diesel” in accordance with CFR and to the satisfaction of the Marine Inspector. 46 § 182.445 (d) Fill and sounding pipes for fuel tanks.

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter firehose is required by § 181.320(b). Existing vessels as of July 22, 2016 have 180 days to comply with this requirement. Aft fire hydrant was missing spanner wrench. Replace to the satisfaction of CFR and Marine Inspector. 46 § 181.310 (d) Fire main and hydrants

The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. These locations must be readily accessible to the intended reader and may include embarkation points, food service facilities, garbage handling spaces, living spaces, and common areas on deck. If the Captain of the Port (COTP) determines that the number or location of the placards is insufficient to adequately inform crew and passengers, the COTP may require additional placards and may specify their locations. Garbage placard was missing from passenger accommodation spaces. Only one placard was on board located in a crew stairway in a restricted area. Install placards where intended readers may be informed to the satisfaction of the Marine inspector. 33 § 151.59 (a) Garbage Placards

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Operator failed to conduct and document monthly portable fire extinguishers IAW NPFA 10. Implement written log to meet CFR requirements. 46 § 176.810 (b) (1) (ii) Fire protection

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). Marine Inspector’s identified 49 passenger lifejackets with failed retroreflective tape. Replace retroreflective tape or replace life jackets to comply with CFR to the satisfaction of the Marine Inspector. 46 § 185.604 (h) Lifesaving equipment markings

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Marine inspector identified main deck and second deck ring buoys with failed retroreflective tape. Replace retroreflective tape or replace ring buoys to comply with CFR to the satisfaction of the Marine Inspector. 46 § 185.604 (i) Lifesaving equipment markings

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions were not mounted at the operating station. 46 § 185.510 (a) Emergency instructions

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Marine Inspector identified 04 waste receptacles that were constructed of combustible materials. Remove and replace to the satisfaction of CFR and Marine Inspector. 46 § 177.405 (f) General arrangement and outfitting.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant OCMI. Marine inspector identified both aft starboard and port mooring lines severely chaffed. Replace both mooring lines to the satisfaction of the CFR and Marine Inspector. 46 § 184.300 Ground tackle and mooring lines.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Provide servicing records for fire fighting equipment. 46 CFR 176.810

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Complete first aid kit to be on the vessel. 46 CFR 184.710

MIS annual report data shall be maintained for a minimum of five years. Obtain and maintain MIS reports. 49 CFR 199.117(a)(2)

An adult lifejacket must be provided for each person carried on board.08 lifejackets need new reflective tape. 15 Lifejackets need to be replaced. Vessel is limited to no more than 130 total persons on board until new lifejackets are onboard vessel.

minimize accidental contact with energized parts. secure galley lights in fixtures. 46 CFR 183.200

make fire pump switch accurate to operation. 46 CFR 183.200

A visual and audible alarm must be provided at the operating station to indicate a high water level. FWD bilge alarm is not indicating at operating station. - 46CFR182.530(a)

Each life jacket carried on board the vessel must be marked in accordance with 185.604. 23 adult and 3 children lifejackets have wasted/ineffective retro-reflective tape. Vessel is limited to 130 total persons until def is rectified. - 46CFR180.71(e)

The PA system must be audible during normal operating conditions. Public address speaker in the kitchen is removed. - 46CFR184.610(b)

Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit. Vessel's captain did not have FCC operator permit. - 47CFR80.155

Vessels must maintain an official log book for the purposes of recording drills, maintenance, incidents, navigation, duties and responsibilities. No log book found on board the vessel during the inspection. 46 CFR 185.420

Light buoy inop, replace floating waterlight. Limited to daytime ops until rectified.

10 Child & 5 Adult lifejackets have wasted retroreflective tape. Replace on each. 10% of the number of persons allowed on board must be provided for child size lifejackets. Limited to carry 100 adult pax.

Vessel must correct mounting installation of pre-engineered fire ext cylinders IAW manufacturers mounting instructions. Cylinders to be mounted using approved mounting brackets to material equivalent to hull material.
