Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 milimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes and in line with the bolthole pattern. 46 CFR 39.2001(j) 40c While on deck, PSCO observed starboard vapor connection flange face reducer missing permanent stud.

Minimum safe manning document requires chief engineer, 2nd engineer, and 3rd engineer. Vessel currently does not meet this requirement due to unfortunate sudden death of 3rd engineer on 22MAY2014 (STCW I/14) (17c).
