Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except duringan emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alterations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of the T-Top alterations to the cognizant [name withheld] to the addition of the T-Top without approved plans it should be removed or a new stability test must be conducted prior to operating with passengers.

A space containing machinery powered by, or fuel tanks for, gasoline must have a ventilation system that complies with this section. The vessel was found to have an inadequate ventilation system for a vessel containing gasoline independent fuel tanks in an enclosed space.

Electrical equipment in spaces, compartments, or enclosures that contain machinery powered by, or fuel tanks for, gasoline or other fuels having a flashpoint of 43.3 degrees C (110 degrees F) or lower must be explosion-proof, intrinsically safe, or ignition protected for use in a gasoline atmosphere as required by 183.530 of this chapter. The vessel's starter batteries are located in a continuous compartment that also houses the gasoline fuel tanks.

The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. The vessel was unable to conduct the exterior "leak" test IAW Sector Honolulu Work Instruction 07 due to not providing the manufacturer's specified maximum allowable working pressure. Subsequently, drills were not conducted.

Each ring life buoy on a vessel must be approved in accordance with §160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. The ring life buoy was found to be damaged and its grab lines to be brittle due to uv damage.

Vessel needs to replace all expired distress signals. In accordance with 46 CFR 180.68.

Vessel needs to have onboard the proper type portable fire extinguisher as required by the vessel's COI. In accordance with 46 CFR 181.500.

Vessel needs to provide a visual indicator for the onboard bilge pump as required in 40 CFR 182.520.

Vessel needs to provide a visual indicator for the onboard high bilge alarm as required in 46 CFR 182.530.

Vessel needs to replace the life buoy detachable clip with a metallic non-corrosive clip as required in 46 CFR 180.70 d (3).

Vessel needs to provide a bight watertight container for its onboard distress signals and label the container as "Emergency Distress Signals" as required in 46 CFR 185.614.

Vessel needs to post emergency procedures for rough water and unresponsive snorkeler as required by 46 CFR 185.510.

Vessel needs to attach lanyards long enough to pick up water from vessel on all fire buckets as required in 46 CFR 181.610.

Vessel needs to have its portable fire extinguishers replaced with two valid B-II fire extinguishers.

Vessel needs to replace the EPIRB release unit.

Vessel needs to install 3 to 6 feet of line with a non-corrosive metallic clip to the vessels float water light.

Vessel needs to stencil vessel name on ring buoy.

Vessel needs to label two child lifejackets at the location where the child lifejackets are stowed.

Vessel operators need to provide proof of holding a valid FCC Marine Radio Operator Permit.

Vessel needs to update its emergency instruction list to incorporate in water emergencies.

Vessel needs to have three stenciled fire buckets onboard.
