Doors leading from the galley of the blue iguana and burger joint (both category 12) were found to have a B-15 door leading to the open pool deck area (category 5 space). Escape routes shall be provided with clear markings for emergency situations. PSCO observed no emergency exit signage leading to escape routes in the port azipod control room. 74 SOLAS 20 II-2/13.1.3 50ac

07105 Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures shall be self-closing and be capable of closing with an angle of inclination of up to 3.5* opposing closure. FSD's 0.3.28, 0.4.17, and 0.5.20 would not release from their magnets upon activation, preventing the from closing. FSD 6.1.04 would not fully close upon testing. 74 SOLAS 20 II-2/9.4.1.1.5.1 10c

07123 Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable extinguishers which have been discharged shall be immediately recharged or replaced with an equivalent unit. A rag was found wrapped around a high pressure water mist nozzle in the Print Shop on Deck 4, completely covering the nozzle discharge. 74 SOLAS 20 II-2/14.2.1.2 10c

The means of escape shall be marked by lighting or photoluminescent strip indicators. PSCO observed extinguished low-location lighting in the forward stairway deck 14. 74 SOLAS 20 II-2/13.3.2.4.1 10c

07199 Category 10 spaces are those where combustible materials can be used for storage. There was a substantial amount of cardboard boxes found within A/C room. 74 SOLAS 20 II-2/9.2.2.3.2.2 10c

11131 Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. The second coxswain on LB# 19 could not explain the operation or prpose of the release gear nor shut off the engine. LB# 20's coxswain and second coxswain could not describe procedures to fight a fire in the engine nor location or operation of the bilge pump. LB#15's second coxswain could not describe or preform a shutting off of the LB's engines. 74 SOLAS 20 III/19.2.2 10c

02105 - The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. On August 10th Vessel reported while underway and arriving to Port Canaveral, FL the vessels Starboard Azipod was experiencing high temperatures in the rotor casing of the Azipod. The temperature rosed from 75 degrees Celcius to 143 degrees Celcius in approximately 10 minutes. The vessels crew could not identify the root cause of the increase of temperature. 74 SOLAS (20) I/11, Code 40ac

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. The crew for Lifeboat #14 could not demonstrate knowledge of lifeboat lowering. 74 SOLAS 20 III/19.2.1 11113

The means of escape, including stairways and exits, shall be marked by lighting or photolumlnescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. Low location lighting on the forward stairway on deck 12 to 14 by the spa were found to be inoperable. Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire fighting system and appliances they may be called on to use. Multiple crew members (10) could not demonstrate proficiency in basic fire fighting knowledge, to include loaction of manual call points, operations of drencher system, and location/operation of fire extinguishers. 74 SOLAS 20 II-2/15.2.2.1 04109

Companies, masters and crew members each have a responsibility to ensure that each crew member can make a knowledgeable and informed contribution to the safe operation of the ship. 04 crewmembers failed to articulate the requirement to call 911 in the event of a medical emergency, contradicting the vessels on board safety instructions. ABOVE THE BULKHEAD DECK, THERE SHALL BE ATLEAST TWO MEANS OF ESCAPE FROM EACH MAIN VERTICAL ZONE OR SIMILARLY RESTRICTED SPACE OR GROUP OF SPACES, AT LEAST ONE OF WHICH SHALL GIVE ACCESS TO A STAIRWAY FORMING A VERTICLE ESCAPE. PSCO OBSERVED DOUBLE-LEAF FIRE DOORS LEADING TO THE AFT STAIRCASE ON THE 02 DECK WAS STUCK SHUT AND WOULD NOT OPEN WITH FORCE APPLIED TO EITHER SIDE. 74SOLAS 20 II-2/13.3.2.2

IT SHALL BE POSSIBLE FOR EACH DOOR TO BE OPENED AND CLOSED FROM EACH SIDE OF THE BULKHEAD BY ONE PERSONONLY. PSCO OBSERVED MAGNETIC DOOR RELEASE FOR FIRE DOORS 1.1.02 DID NOT FUNCTION WHEN PUSH BUTTON RELEASE WAS ACTIVATED. FIRE DOORS 3.2.13 WOULD NOT LATCH WHEN ACVIVATED TO CLOSE 74 SOLAS 20 II-2/9.4.1.1.4

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSC Examiners during life boat operational testing discovered #15 life boat could not engage the starboard engine.

While operating within an emission control area, the sulphur content of fuel oil used on board ships shall not exceed the following limits: 0.10% m/m on and after 1 January 2015. Vessel burned non-compliant heavy fuel oil without using exhaust gas cleaning system in the North American ECA on 21 April 2020. Marpol VI/14.4.3 10c

74SOLAS (2014) III/7.3 - An immersion suit or anti-exposure suit of an appropriate size, shall be provided for every person assigned to the marine evacuation system party. Immersion suits were not available to marine evacuation system team members.
