Every ship of 12 m or more in length overall and fixed or floating platforms shall display placards which notify the crew and passengers of the discharge requirements of regulation 3,4,5 and 6 of this annex. Resolution MEPC 219(63) requires where ships carries passengers, placards also should be placed in prominent places where passengers are accommodate and congregated. These include cabins, all deck areas for recreational purposes open to passengers. MARPOL placard were not found on any open deck open to passengers including deck 5 crew observation area sometimes utilized by passengers. MARPOL 2017 Annex V/10.1

The following functional requirements shall be met: escape routes shall be maintained in safe conditions, clear of obstacles. Exterior escape doors leading to the weather decks on deck 5 and 4 were seized shut and required extreme force to open. 74SOLAS 20 II-2/13.1.2

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more then 300mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers/crew to identify the routes of escape and readily identify the escape exits. Additional, escape route signs and fire equipment location marking shall be of photoluminescent material or marked by lighting. Aft crew elevator leading on deck 6 through 4 is blocked by trolly storage. 74SOLAS 20 II-2/13.1.2

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, remote release sliding or power operated doors shall be equipped with an alarm that sounds at least 5 s but no more than 10 s after the door is released from the central control station and before the door begins to move and continues sounding until the door is completely closed. Fire door 6417 failed to close within 10 seconds. 74SOLAS 20 II-2/9.4.1.1.5.10

The load line ring, lines and letters shall be painted white or yellow on a dark background or in black on a light background. They shall also be permanently marked. The deck line associated with the load line is painted the same blue blue color as the hull. ILLC 8

In multi engine installations which are supplied from the same fuel source , means of isolating the fuel supply and spill piping to individual engines shall be provided. #1 and #3 generator fuel supply quick closing valves did not close upon first activation attempt. 74SOLAS 20 II-2/4.2.2.5.2

Fire hoses shall be of non-perishable material and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and necessary couplings. Hoses specified in this chapter as fire hoses shall, together with any necessary fittings and tools, be kept ready for use... 03 fire hoses (ivo FE 0D.05.07, Hose: 0C.02.24 & 0C.01.35) spanner wrench were missing at the fire hose station. 74SOLAS 20 II-2/10.2.3.1.1

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire hose station (ivo Cabin A002) required a cross key for access, making it not available for immediate use. 74SOLAS20 II-2/14.2.1.2

Containers, brackets, rack, and other similar stowage locations for life saving equipment shall be marked with symbols in accordance with the recommendations of the organization, including the devices stowed in that location for that purpose. The lifebuoy located stbd side bow has wrong placard. The lifebuoy indicates it should have an attached line but, the LSA plan shows a lifebuoy with no line. 74SOLAS20 III/20.10

Waste receptacles shall be constructed of non combustible materials with no openings in the side or bottom. Combustible waste receptacles were found for the collection of plastic and other combustible waste at the recycling center on deck 1. 74SOLAS20 II-2/4.4.2

An International Oil Pollution Prevention Certificate shall be issued, after an initial or renewal survey to any ships of 400 gross tonnage and above which are engaged in voyages to ports or offshore terminals under the jurisdiction of other parties to the present convention. PSCO observed the throughput of the OWS missing on the IOPP. MARPOL 2017 Annex I/7.1 16ac

Fire-fighting equipment and appliances shall be kept in good working order and readily available for immediate use. PSCO observed suspected dry rot of CO2 hoses in main CO2 room. 74 SOLAS 20 II-2/14.2.1.2 16ac

The company shall ensure that; valid documents are available at relevant locations. PSCO observed IOPP issued in 2021, endorsed in 2022 and 2023 and did not have a listed throughput of their OWS. As seen in subsequent deficiency of IOPP issued by class. External audit required. 74 SOLAS 20 IX ISM code A.11.2.1

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed non-combustible container in collection area for combustible materials outside of the garbage room. 74 SOLAS 20 II-2/4.4.2 10c

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, release mechanism shall be so designed that the door will automatically close in the event of disruption of the control system or central power supply. The following fire doors would not shut when released: 9.5.04, 9.5.22, D.2.14, A.3.78, 1.5.27, 4.3.43

All life-saving applicnes and arrangement shall comply with the applicable requierments of the code. The engine system and starting aids shall start the engine...within 2 minutes of commencing the start procedure. Upon testing, lifeboat #4 did not start. 74SOLAS(20) III/34 LSA Code 4.4.6.2 Action code: 17ac

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Multiple stariway guides on various decks in the forward stariway were unable to provide the correct passanger muster station utilizing the passanger cabin referece card located on the starway guide's lifejacket. 74SOLAS(20) III/19.2.1 Action Code: 17c

Below the bulkhead deck, two means of escape, at elast on of which shall be independent of watertight doors, shall be provided from each watertight compartment or similar restricted space or group of spaces. PSCO observed a space in the linen store/tailor shop on C-deck forward, fire zone 6, that appeared modified. The space did not provide tow means of escae and the escape routes provided did not match the apprved evacuation plan. 74SOLAS(20) II-2/13.3.2.1.1 Action Code: 50ac

The door directly outside the Stuyvesant meeting room, in indicated as a means of escape to the muster station with IMO symbols but does not have escape route signage. 74SOLAS(20) II-2/13.3.2.5.1 Action Code: 50C

The following functional requierments shall be met: escape routes shall be maintained in safe conditions, clear of obstables. Carts and trash cans were observed IVO the strway outside of the garbage sorting room on deck 1, impeeding the means of escape. 74SOLAS(20) II-2/13.1.2 Action Code: 10c

Doors and door frames in "B" class division and means of securing them shall propvide a method of closure whcih shall have resistance to fire equivelent to that of the disvision. B-15 door IVO FSD 2.3.90 does not latch. 74SOLAS(20) II-2/9.4.1.2.1 Action Code: 10c

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power. Eight embarkation lights on the starboard side and one on the port side, aft were extinguished. 74 SOLAS 20 III/11.4

In the ship's medical center, various furniture items including couches/chairs/filing cabinet were obstructing low location lighting. 74 SOLAS 20 II-2/13.3.2.5.1

Deck 4, aft, crew elevator shaft was obstructed by several steward carts, allowing for less than 800mm of escape. Additionally, A deck stairwell adjacent to electricians office had a utility cart blocking means of escape. 74 SOLAS 20 II-2/13.1.2

Prior to being assigned shipboard duties on board passenger ships, seafarers shall have completed the training required by paragraphs 4 to 7 below IAW their capacity, duties and responsibilities. During abandon ship drill, PSCO observed several stairwell guides throughout all three stairtowers who were unfamiliar with the procedures for allowing passengers to return to their staterooms during the general emergency alarm signal. STCW V/2.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and pentrations in these divisions. A-60 insulation was missing in the aft auxiliary machinery space IVO WTD C.7. 74 SOLAS 20 II-2/13.2.1.1.1

The capacity of the generating sets shall be such that in the event of any one generating set being stopped it will still be possible to supply those services necessary to provide normal operational conditions of propulsion and safety. Upon manual shut down of DG#1, DG#3 became overloaded and shut down causing a temporary black out and loss of propulsion. 74 SOLAS (20) II-1/41.1.2

Means of escape, including stairways and exits, shall be marked by lighting or photo luminescent strip indicators. PSCO observed security screening equipment was blocking photo luminescent strips in A deck forward crew corridor in vicinity of fire screen door A.5.17. SOLAS 20 II-2/13.3.2.5.1

Ensure slot machines located in front of embarkation windows meet the requirements of 5 liters per minute application rate for A-60 boundary adjacent to STBD embarkation station.

All lifeboats shall be fitted with sufficient watertight lockers for the storage of small items of equipment, water, and provisions. Every lifeboat foudn with plastic totes of P.P.E in way of seating arrangement.

Fire doors... shall satisfy the following requirements... double-leafed doors equipped with a latch necessary for their fire integrity shall have a latch t hat is automatically activated by the operation of the doors when released by the system. PSCOs noted 8 fire doors that were unoperable due to the sequencing bars not being adjusted properly. Fire doors: A.3.43, A.3.45, A.3.70, A.4.48, 5.2.47, 10.2.35, 9.4.34. 74 SOLAS 14 II-2/9.4.1.1.5.12

Above the bulkhead deck there should be at least two means of escape from each main vertical zone. A/C room spanning from decks 4, 5, and 6 is missing secondary means of escape on deck 4. Fire control plan lists emergency escape through a ladder connecting all three decks, however this ladder does not lead to deck 4 as indicated by the plan. 74 SOLAS 14 II-2/13.2.2

1430- Main and Auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. RPM table not present in emergency steering gear compartments. Emergency Propulsion Station. 74SOLAS (14) II-1/31.1

0750- Doors and door frames in "B" Class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions. Several "B" Class doors on Deck 9 galley were wedged open, compromising B class boundary. 74SOLAS (14) II-2/9.4.1.2.1

2025- Each Abandon ship drill shall include a mock search and rescue of passengers trapped in their state rooms. Crew members responsible for clearing rooms on deck 4 port side forward did not properly carry out their duties to ensure that the rooms were cleared 74SOLAS (14) III/19.3.3.1.8

0680-PSCO observed 04 embarkation lights were not working properly on the ships port side. 74SOLAS 14 iii/11.4

1410- The condition of the ship and its equipment shall be maintined to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons onboard. Vessel reported intermittent loss of RPM control on both port and starboard azipods and are receiving an RPM alarm when operating in bridge control. -74SOLAS(14) I/11(a)

74SOLAS(2014) Ch. III/20.4 - Falls used in the launching shall be inspected periodically with special regard for areas passing through sheaves, and renewed when necessary due to deterioration of the falls or at intervals of not more than 5 years, whichever is the earlier. Vessel discovered deterioration of falls on all 6 tender boats.
