"The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations." The vessel was found to have not followed procedures to ensure that the pilot transfer arrangement was safe and effective. An internal safety audit is required within 3 months.

"All arrangements used for pilot transfer shall efficiently fulfill their purpose of enabling pilots to embark and disembark safely." The vessel's pilot transfer arrangement had found to have stanchions on the landing platform that were not locked securely in place by safety pins.
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign

Resolution: PSCO observed proper pilot landing platform arrangements during PSC B exam.
