In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to this acceptable to the organization (IEC 60092-5021999). Three cable containment pipes, starboard side just forward of house, found without bonding straps, drain plugs removed. The cable pipe/junction boxes and house have an open means to allow cargo vapors to enter the pipe.

Where the atmosphere in double-hull spaces cannot be reliably measures using flexible gas sampling lines/hose, such spaces shall be fitted with permanent gas sampling lines. Found 5P ballast tank fixed gas sampling line disconnected, a flexible hose is installed: the flow rate and vacuum capabilities are in question.

Resolution: Received class report with misidentified line on vessel.
Any ship of 10,000 GT and above shall be fitted with oil filtering equipment (which) will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. In addition, it shall be provided with alarm arrangements to indicated when this level cannot be maintained. The system shall also be provided with arrangement to ensure that any discharge of oily mixture is automatically stopped when the oil content of the effluent exceeds 15 ppm. Vessel self-reported that the onboard oily water separator is inoperable. MARPOL I/14.2
