The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids for this purpose, functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed port side MGO settling tank leaking from a fitting near the deck into an open catch pan with about one gallon of fuel in the pan. Additionally, PSCO observed FO #2 circulating pump leaking. 74 SOLAS (20) II-2/4.1.1 30ac

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: Thermal insulation of boundaries shall have due regard to the fire risk of the space and adjacent spaces. PSCO observed missing insulation on overhead by port MGO settling tank. This boundary is identified as “A-60”. 74 SOLAS (20) II-2/9.1.2 17ac

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed hose clamps and other evidence of temporary repair on fresh water line feeding hyper-mist system, as well as on an unknown pipe above MGO settling tank (port) in the engine room. 74 SOLAS (20) 1/11a 17ac

PSCO observed multiple faults on the hydraulic power unit remote controller for all cargo and ballast valves. 74 SOLAS (20) VII /10.1 IBC CODE 1.5.3.1 17 AC

Each tank ship with a vapor collection system must be fitted with a pressure sensing device located as close as practicable to the vessels vapor connection, that measures the pressure in the main vapor collection line, which has a high pressure and a low-pressure alarm that gives an audible and visible warning on the vessel where the cargo transfer is controlled. Vessel does not have a hand pump for testing, PSCO was unable to verify operation of high and low pressure alarms on the VECS. 46 CFR 39.2013(b)(1) 17 AC

Electrical equipment cables and wiring shall not be installed in the hazardous locations unless it conforms with the standards not inferior to those acceptable to the organization. PSCO observed grounding wire to the tank radar for the 1 port cargo tank disconnected. 74 SOLAS (20) VII/10.1 IBC CODE 10.1.4 10C

The conditions of the ship and its equipment shall be maintained to conform with the provisions of theCode to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons onboard or without presenting an unreasonable threat of harm to the marine environment. PSCO observed active alarm on power pack #4 due to low start battery voltage. Additionally, PSCO observed inoperable drive pressure gauge for #4 starboard Framo pump. 74 SOLAS (20) VII/10.1 IBC Code 1.5.3.1

Electrical equipment, cables and wiring shall not be installed in the hazardous locations unless itconforms with the standards not inferior to those acceptable to the Organization. PSCO observed holes in the electrical run to 4S Framo pump sensor (discharge). Addtionally, PSCO observed gland for explosion proof light aft of tank slop S not properly packed. 74 SOLAS (20) VII/10.1 IBC Code 10.1.4

Each tankship with a vapor collection system must be fitted with a pressure-sensing device, located as close as practicable to the vessel vapor connection. PSCO observed fitting for starboard aft VCS pressure sensor severely corroded. 46 CFR 39.2013

Additionally, PSC observed multiple hyper-mist nozzles blocked with rust and saltwater buildup, preventing any flowto the nozzle above the incinerator and restricting flow above boiler. 74SOLAS(2004) II-2/14.2.1.2

Firefighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed local controls and pilot cylinder for the emergency diesel generator fixed CO2 system seized shut. Additionally, PSCO observed, activator valve handles painted and seized in the closed position. 74SOLAS(2004) II-2/14.2.1.2

PSCO observed severe wastage to electrical runs, including flexible conduit within the hazardous area indicated in the vessels approved hazardous location plan specifically, -Temp sensor (discharge) to slop port Framo pump, starboard slop junction box, two starboard tank radar, one starboard temperature sensor. -bolts missing on Ballast Water tank six starboard tank level sensor, bolt missing on temperature sensor three port, bolt missing ballast wing tank two port tank level sensor. -bonding strap above tank one starboard, ground wires disconnected to forward, starboard explosion proof light, and to deck crane motor controller -Finally, PSCO observed cracked explosion proof light on the forward port side forecastle bulkhead. PSCO traced to the intrinsically safe circuits back to the electrical panel, located in cargo control room, and observed faults on four isolation devices compromising the circuit. 74SOLAS(2020) VII/10.1 IBC 10.1.4

For tankers of 20,000 tons deadweight and upwards the protection of the cargo tank shall be achieved by fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code. PSC observed vessel's inert gas generator inoperable. The crew identified the gas regulating valve was not functioning properly on 24 January 2024, rendering system unable to provide inert gas at or below 5% oxygen. Crew notified Company without rectification. 74SOLAS(2004) II-2/4.5.5.1.1

Bilge Wells in periodically unattended machinery spaces shall be located and monitored in such a way that the accumulation of liquids is detected at normal angles of trim and heel. PSCO observed crew unable to activate bilge alarms. 74SOLAS(2004) II-1/48.1

The function of the fixed fire detection and fire alarm system shall be periodically tested to the satisfaction of the administration by means of equipment producing hot air at the appropriate temperature, or smoke or aerosol particulates having the appropriate range of density or particle size, or other phenomena associated with incipient fires to which the detector is designed to respond. PSCO observed crew does not have required testing equipment on board to test flame detectors or manually operated call points. 74SOLAS(2004) II-2/7.3.2

A chemical tanker shall comply with the requirements of the international bulk chemical code and shall, in addition to the requirements of regulation I/8, I/9, and I/10, as a applicable, be surveyed and certified as provided for the code. Ships carrying toxic or flammable products or both shall be equipped with at least two instruments designed and calibrated for testing for the specific vapors in question. PSC observed fixed gas detection system inoperable, portable detectors for toxic cargo on board were not available for toluene or xylenes. 74SOLAS(2020) VII/10.1 IBC 13.2.1

Doors required to be self closing shall not be fitted with holdback hooks. PSCO observed both A-class self closing doors to galley as indicated on vessels approved fire control plan, fitted with holdback hooks. Additionally, PSCO observed self closing mechanism removed from officer recreation room, and A-class self closing fire door to "B" deck cable duct, unable to latch as intended. 74SOLAS(2004) II-2/9.4.2.2

Where A-Class divisions are penetrated… penetrations shall be suitably insulated by extension of the insulation at the same level of the division. PSCO observed insulation in A-class division separating "A" deck and "B" deck cable ducts, completely missing in two cable runs and partly missing in one cable run. 74SOLAS(2004) II-2/9.3.1

PSC observed oil accumulation below the flywheel of all the generators, fuel oil pooling in the catch tray of the number two generator fuel filters, and on top of the boiler. 74SOLAS(2004) II-2/4.1.1

The conditions of the ship and its equipment shall be maintained to conform with the provisions of the Code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons onboard or without presenting an unreasonable threat of harm to the marine environment. PSCO observed vessels electronic tank radar displaying inaccurate readings, particularly tank six starboard containing mixed xylene, a type two cargo, containing over 3000 m³ of product. Crew confirmed via UTI, Actual level is 2998 m³. Additionally, PSO observed temperature sensors displaying inaccurate readings, particularly tank one starboard was 89°C and tank six starboard was reading -67°C. 74SOLAS(2020) VII/10.1 IBC 1.5.3.1

The inert gas supply main shall be fitted with branch piping leading to each cargo tank. Branch piping for inert gas shall be fitted with either stop valves or equivalent means of control for isolating each tank. PSCO observed the isolation valve to one port cargo tank taken apart at the valve gearbox. The valve is inoperable, and cannot be determined to be open or closed. 74SOLAS(2020) II-2/4.5.5.1.1 FSS CODE (07) 15.2.3.2.2

After any survey of the ship under Article 14 has been completed, no change shall be made in the structure, equipment, arrangements, material or scantlings covered by the survey, without the sanction of the Administration. PSC observed load line painted with a tropical fresh, fresh, tropical, summer, and winter load line as indicated on certificate number 2223778/LL1 with a deadweight 50,129 metric tons, however, the last activity and correspondence from class indicates the vessel should be operating with a load line with just a freshwater mark and a deadweight of 34,999 metric tons as indicated on certificate number 2223778/LL4. ILLC 66 Article 15

Every ship to which this Annex applies shall be provided with a Cargo Record Book, whether as a part of the ship's official logbook, or otherwise, in the form specified in appendix II to this Annex. PSCO observed the cargo record book being in another format, not matching appendix II. MARPOL II/15.1

Stowage of cargo samples. The stowage space shall be: - cell-divided in order to avoid shifting of the bottles at sea. PSCO observed cargo sample storage locker full with additional samples stowed above the cell enclosure. 74SOLAS(2004) VII/10.1 IBC 16.5.2

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed 80 nitrogen bottles to the deck in the cargo area. Some bottles were missing the protective caps. 46CFR 147.60(b)(1)

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The technical and or operational nature of deficiencies 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 13,16 and 17 provide objective evidence of a serious failure of the implementation of the ISM code. An external audit report satisfactory to the OCMI must be submitted prior to the clearance of this deficiency. 74SOLAS(20) IX/3.1 ISM Code 10.1

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. PSCO observed no flange stud on the starboard forward vapor connection. 46 CFR 39.2001(j)

The conditions of the ship and its equipment shall be maintained to conform with the provisions of theCode to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons onboard or without presenting an unreasonable threat of harm to the marine environment. PSCO observed valve position indicator for 2 port, 3 port, 4 port and stbd, 5 stbd, and stbd slop cargo tank valves on the hydraulic power unit remote controller console in CCR. 74 SOLAS (20) VII/10.1 IBC Code 1.5.3.1

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed disconnected ground wires on explosion proof light aft of stbd slop tank, stbd embarkation deck emergency light, and light on port side of upper funnel. Additionally, PSCO observed multiple wire runs in the engine room with compromised glands exposing the conductors to include motors on sewage treatment plant and HFO/LO purifiers and most tank level sensors for storage tanks within the engine room. 74 SOLAS (04) II-1/40.1.3

The machinery, boilers, and other pressure vessels associated piping systems and fittings shall be of a design and construction adequate for the servie for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board due regard being paid to moving parts, hot surfaces and other hazards. Vessel suffered a reduction in propulsion due to issue with steam bypass valve on main engine fuel oil heaters. 74 SOLAS (04) II-1/26.1
