The machinery, boilers, and other pressure vessels, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minumum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected, and the environmental conditions on board. The vessel was unable to operate the main engines in bridge control due to ICU malfunction.
Action required: 60 - Rectify deficiencies prior to movement

Class A-0 doors located on F and B deck leading to stairwell have damaged and / or missing gaskets resulting in insufficient smoke / flame control as designed.

Resolution: The A-0 class door w/ frame on F-deck leading to stairwell has completely been replaced w/ new one. The door frame packing attached to the A-0 class door on B-deck leading to stairwell has newly been installed. Materials mentioned have been approved.
