02-The company should establish procedures for key shipboard operations. PSCO observed the pilot ladder tied directly to the cargo hold using a fiber rope. The vessel's safety management system states that pilot ladders should be secured to deck strong points. The vessel's crew stated the pilot ladders on board were only able to be used properly when attached directly to the cargo holds. 74 Solas (2020) IX/3.1 ISM Code 7

Resolution: Received Class report attesting that the pilot ladder was tied/secured to the original secure point (on deck eye point) by crew with satisfactory result.
