The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship- based and shore-based. As required by technical manual TEC-0140.4I. Numbered seals are to be used at the OWS overboard valve and flange bolts. Chief engineer maintaiins a seal log book for this purpose. PSCO observed the OWS overboard valve flange bolts were not capable of being sealed, and the seals that were placed on the flange bolts on the OWS overboard piping were not logged in the seal log book. 74 SOLAS (20) IX Reg 3.1 / ISM Code 2.2

Resolution: PSCO verified the crew installed a seal on the supply-side flange of the OWS overboard valve, and all 5 seals on the valve and associated piping were properly logged in the Seal Log.
