All lifesaving appliances and arrangements shall comply with the applicable requirements of the code. LSA Code states, life jacket lights and whistles shall be selected and secured to the lifejacket in such a way that their performance in combination is not degraged.
PSCO observed several lifejackets on board to have missing or damaged light holders, leaving a torn opening in the lifejacket. Lights that were affixed were not installed as per manufacturers instructions. Provide corrective action report with concurrence from certificate issuing authority.
