Cargo handling equipment shall be maintained by the tank vessel's personnel in accordance with the regulations. Vessel experienced a fire on the cargo discharge pump causing damage and rendering the cargo pump inoperable. Repair or replace affected cargo pump and affected materials. The vessel is prohibited from all cargo operations until repairs are made to the satisfaction of the USCG.

As a documented vessel, hull required to be permanently marked with vessel name on stern and bow. The vessel's bow (port and starboard sides) were not marked. Vessel's stern had the name of the vessel (and hailing port) on a metal plate suspended from cargo piping by two ends of a piece of line. Bow and stern required to be properly marked in accordance with 46 CFR 67.123.

46 CFR 29.2001(h) requires the last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection to be painted in the sequence of red/yellow/red AND labeled with the word “VAPOR” painted in black letters. The word "VAPOR" was worn and/or missing on the vessels vapor control system vapor piping. The vessel was also equipped with a "cross-over dummy" pipe. According to oil transfer procedures the cross-over pipe is for vapor transfers. Each end of the pipe was fitted with a stud, require for VCS connections. Cross-over piping did not have the required wording or the required painted red/yellow/red bands.

33 CFR 155.750 requires the vessel's oil transfer procedures (OTPs) to include certain cargo information for products carried by the vessel. The OTPs reference an applicable material safety data sheet for the current cargo carried. The vessel did not have an MSDS for the previous cargo carried (vessel not gas free). Required an appropriate MSDS be placed on board vessel.
