The inert gas system shall be capable of inerting, purging, and gas freeing empty tanks and maintaining the atmosphere in cargo tanks with the required oxygen content. The inert gas generator onboard was inoperable and unable to inert cargo tanks. SOLAS (14)II-2 5.5.3.1 17ac

The company and ship shall comply with the requirements of the international safety code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules. In meeting these requirements, the company should ensure that 1) any nonconformity is reported 2)records of these activities are maintained. PSCO observed an inoperable Inert Gas System. Crew reported that system had been inoperable since 13Mar21, however, PSCO observed objective evidences that system had been inoperable as early as 11Mar21. No formal report was made to the company. In addition, PSCO observed maintence log for EEBD's in engine room prefilled or missing indicating that the records were falsified. An internal ISM audits is required of the vessel's reporting and record keeping procedures to be completed within 3 months of this report. SOLAS (14) IX-3.1 705ac

06105- In addition to the requirements in paragraphs 5.7.1 and 5.7.2, oil tankers of 20,000 tonnes deadweight and above, constructed on or after 01 January 2012, shall be provided with a fixed hydrocarbon gas detection system complying with the Fire Systems Code. PSCO observed multiple sampling ports displaying a flow fault. Complete needed repairs prior to loading cargos requiring a gas detection system. SOLAS 74(a)2014(cons) II-2/4.5.7.3.1 A

99901- A vessel owner or operator required by 155.1035, 155.1040 or 155.5035 to have a response plan shall conduct exercise as necessary to ensure that the plan will function in an emergency. Qualified Individual notification exercises which must be conducted quarterly. Ship did not conduct QI notificaion exercise. 33 CFR 155.1060(a)(1) 10C

2510: Vessel must maintain a Ballast Water Management and Bio-Fouling plan that has been developed specifically for the vessel that will allow those responsible for the plan to understand and follow. The vessel currently does not follow the Ballast Water Mangement Plan and the Bio-Fouling Management Plan is incomplete and does not meet requirement under 33CFR2050(g), 10C.
