A fan was wired into the spare breaker (9-406 EP) in the distribution panel near the emergency switchboard in the EDG space. During preparation and launching, the survival craft, its launching appliance, and the area of water into which it is to be launched shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulations II-1/42 or II-1/43, as appropriate. Embarkation lighting for aft liferafts found inoperable. 74SOLAS(97) III/15.7

The safety-management system shall be maintained in accordance with the provisions of the International Safety Management Code. 10.1 - The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. 10.2 In meeting these requirements the Company should ensure that: .1 inspections are held at appropriate intervals; .2 any non-conformity is reported with its possible cause, if known; .3 appropriate corrective action is taken Objective evidence shows that engine hours were not transfered to the NSE maintenance system, and hour based maintenance is not being properly tracked. No non-conformity was reported per the code. Company is to conduct an investigation to determine the root cause, develop and implement any corrective action into the SMS to prevent future occurrences. Require additional SMC verification be conducted focusing on maintenance tracking and promulgation.

Pinhole leak discovered on inlet water piping between skin valve and sea chest at welded junction from 22" pipe from sea chest to 5" pipe to saltwater evap #1. Temp repairs made to reduce water ingress. Vessel shall immediately return to Rota, Spain at safe speed to complete permanent repairs prior to departure from port.

Each watertight subdivision bulkhead, whether transverse or longitudinal shall be constructed in such a manner that it shall be capable of supporting with proper margin of resistance, the pressure due to max head of water which it might have to sustain in the event of damage to the ship. While loading the port side vehicle access door at 5-71-2 was struck and damaged. All ships of 500GT shall have a gyro compass heading or other means to determine and display their heading by shipborne non magnetic means. The gyro repeater at the helm is not working. Make repairs to system by replacing fried card for helm gyro repeater. All other gyro repeaters are working. Repairs made to the satisfaction of ABS. 74 Solas (20) V/19.2.5.1

Vessel’s sliding watertight door #571-1 located on the D4 cargo deck will not lock in place. Crew is able to manually manipulate the solenoid to close and lock the watertight door. Vessel is to keep door closed and locked while underway until repaired.

Machinery spaces of category A containing internal combustion machinery shall be provided with one of the fire-extinguishing systems required by paragraph 1.1. (Paragraph 1.1.1) a gas system complying with the provisions of regulation 5. (Regulation 5.1.4) Means shall be provided to close all openings which may admit air to or allow gas to escape from a protected space. Emergency Generator room contains an opening in overhead allowing for engine exhaust pipping exit with no means of closure.

The machinery, boilers and other pressure vessels, associated piping, systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being to paid to moving parts, hot surfaces, and other hazards. Auxiliary boilers safety valve handles have springs wrapped around them preventing proper operation.

Simple operating instructions with a block diagram showing the change-over procedures for remote steering gear control systems and steering gear power units shall be permanently displayed on the navigation bridge and in the steering compartment. The steering compartment was missing operating instructions. 74SOLAS(20)V/26.3.1

During an On-board Condition Inspection Enhanced and Dock Trial (OCI-E, DT) 02-06 March 2020. It was discovered a) That the ship did not have adequate LP CO2 fire suppression agent b) The cargo WTD 4-71-1 [cargo hold C4] and WTD 7-71-2 [cargo hold F3] failed to latch. -- As per the ship's Safety Management System these items CO2 Plant and Watertight Doors are considered Critical Equipment (ISM Code 10) -- Whenever an a defect is discovered which affects the safety of the ship the master or owner of the ship shall report at the earliest opportunity to the recognized organization responsible for issuing the relevant certificate. Provide Proof that the Authorized Classification Society was notified of these major findings

Lifeboat ladders attachment points were severly wasted, gauge to sat of Class or replace to same.

Recue boat did not start and the battery needed to be charged before it would even turn over.

Vessel self-reported degraded AIS on board vessel. AIS must be maintained in effective operating condition. Repair AIS to the satisfaction of class prior to departure. (74 SOLAS (2014) V/19.2.2.4)

74 SOLAS (14) V/20.2 To assist in casualty investigations, cargo ships, when engaged on international voyages, shall be fitted with a VDR which may be a S-VDR in the case of cargo ships 20,000 GT and upwards constructed before 1 July 2002. Vessel reported S-VDR circuit board is faulty and requires replacement.

Main steering gear and rudder stock shall be capable of putting the rudder over from 35 degrees on one side to 35 degrees on the other side. The steering gear will not achieve 35 degrees using bridge controls. SOLAS (97) II-1/29.3.2

Every crewmember shall participate in at least one abandon ship drill and one fire derill every month. When new crew is engaged, drills shall be held before sailing. Upon activation, conduct drills in the prescence of the attending CG marine Inspector.

Each lifeboat shall be launched and maneuvered in the water by its assigned operating crew at least once every three months. Provide log showing that all boats have been launched as required or launch all lifeboats upon activation in the presences of the attending CG Marine Inspector

74SOLAS 81 Amend II-I/44 Each emergency generating set arranged to be automatically started shall be equipped with starting devices appreved by the administration with a stored energy capability of at least three consecutive starts. Vessel start air is inoperable.

Vessel is unable to access 02 fuel tanks due to faulty motorized valves. Vessel shall repair and demonstrate satisfactory operation to the satisfaction of the attending ABS Surveyor prior to departure

Enclosed spaces that are specially suitable for vehicles must be fitted with a fire detection and alarm system. Fire detection system has (9) faults.

Each watertight door in a required subdivision bulkhead must have an indicator light installed in the pilothouse and each other vessel operating station from which the door is not visible. Several watertight door indicating lights installed in the pilothouse are not functioning to show whether the doors are opened and closed.

When fire drills are held, official logbook entries must include identification of crewmembers participating in drills. Vessel crew failed to identify crewmembers participating in fire drills in the official logbook.

The integrity of a stairtower shall be maintained by "A" class doors at every level. The doors shall be of self closing type. There are 17 doors not closing properly.

Drills must as far as practicable, be conducted as if there were an actual emergency. Vessels crew failed to adequately demonstrate proficiency in the use of fire fighting equipment and recognition or initial actions necessary, while conducting 2 fire drills.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery or equipment shall be made without the knowledge of OCMI. As per 46CFR56.50-55a, 2 power driven pumps are required. Currently installed diaphragm pump does not meet standards as a bilge pump.

Each lifeboat must be launched with its assigned operating crew aboard and maneuvered in water once every 3 months.

Replace or Repair scuttle between D and E deck.

Audible and Visual alarms on Watertight door panel inoperable on the bridge.

Replace 3 50' fire hoses.
