A fireman's outfit shall consist of: personal equipment comprising: An axe to the satisfaction of the administration. During fire drill, firefighters were not issued fire axes. One axe was presented that was not an insulated fire axe. Provide approved fire axes, such as those required by FSS 3/2.1.1.1.5, or equivalent for each fireman's outfit to the satisfaction of ABS. 74 SOLAS (97) II-2/17.1.1.5

"Trouble" alarm became active during operational testing of the fire detection system and this alarm could not be cleared prior to completion of the COI Exam.

Every oil tanker of 150 gross tonnage and above and every ship of 400 gross tonnage and above other thanan oil tanker shall be provided with an oil record book part 1 (machinery space operations). MI noted in past Oil Record Book a letter from the company stating a 3rd Engineer eronously entered multiple entries into Oil Record Book that did not align with tank ullages or quantities transferred between tanks.

Vessel experienced loss of propulsion due to a cracked turbocharger on one MDE and inability to clutch-in on another MDE. A vessel enrolled in the ACP program must comply with applicable class rules for the proper operational status of propulsion machinery. VSL shall not proceed beyond the boundary line until reapirs on the 02 inoperable MDEs are effected to the satisfaction of class and a preliminary report is provided to the CG.

Every lifeboat shall have food ration as described in paragraph 4.1.5.1.18 totalling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate; these rations shall be kept in airtight packaging. Inspectors found several rations to have lost their airtight packaging. Vessel to ensure each person the lifeboat is permitted has 10,000 kJ. 74SOLAS(09/14)111/34 LSA Ch. IV 4.4.8

The machinery, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended. A hose on the OWS is leaking. Replace hose IAW manufacture specifications. 78 SOLAS 04 Ch. II-1 Reg. 26.1

Fire detection and alarm systems shall be kept in good order so as to ensure their required performance if a fire occurs. The fire alarm panel in the engine control room is showing a trouble alarm. Correct the fault that is causing the trouble alarm. 78 SOLAS 04 CH. II-1 Reg 14.2.1.1

In cargo ships, ventilation fans shall normally be run continuously whenever vehicles are on board. Where this is impracticable, they shall be operated for a limited period daily as weather permits and in any case for a reasonable period prior to discharge, after which period the ro-ro or vehicle space shall be proved gas-free. One or more portable combustible gas detecting instruments shall be carried for this purpose. The combustible gas detector in Hold #5 has a sensor fault. Repair fault to ensure proper system operation. 78 SOLAS 04 CH. II-1 Reg 20.3.1.2.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The refrigerant detector is showing a trouble alarm. Correct the fault that is causing the trouble alarm. 78 SOLAS 04 CH. I Reg. 11.a

Maintenance, testing and inspections of life-saving equipment shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring reliability of such appliances. Vessel's lifeboats on port and stbd side are making contact with steel supports on the launching davits, causing damage to the hulls.

The record under paragraph (a) of this section must be prepared at the time of the operation, certified as correct by the master or person in charge of the ship, maintained on the ship for 2 years following the operation and made available for inspection by the Coast Guard. Vessel's garbage receipt dates for discharge to shore and log book dates, as well as categories of garbage sent ashore do not match.

(1)The minimum sound-pressure levels for the emergency-alarm tone in interior and exterior spaces must be a sound level of not less than 80 dB(A) measured at 10 feet on the axis; and (2) At least 10 dB(A) measured at 10 feet on the axis, above background noise level when the vessel is underway in moderate weather unless flashing red lights are used in accordance with 46CFR113.25-10(b). Working areas throughout the engine room were unable to hear the audible general alarm when no visual alarm indicators were present.

Where the general emergency signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm that has sufficient intensity above the background lighting that would alert personnel in the space. Working areas throughout the engine room had missing or inoperable lights where the audible alarm is unable to be heard.

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. The vessel was unable to show proof that the raw-water expansion joints had been replaced within the previous 10 years.

Tested fire main and noted several fire station hydranty valves leaking by charging hoses. Overhaul valves at fire stations 10, 51, 102, 112, 136, 140, 143, 144 & 146 within 30 days to class satisfaction.

Port side rescue boat air bladders deflatted. Reinflate prior to sailing to maintain operational readiness Prior to departure.

2 life buoy plugs on port side main deck failed. Renew plugs to restore watertight integrity.

02 Engineroom forced ventilation vouvers (C5 P/S) tested inoperable. Vessel directed to repair, survey, and prove proper operation of all forced vent louvers servicing the engineroom. 74 SOLAS (2001) II-2/5.2.2.2

As noted by ABS reports there are several outstanding items have been extended and vessel is authorized to sail in support of National Defense Operations. Upon completion of Military mission and unless otherwise specified the following OSRs are to be repaired at next US port after sailing foreign to satisfaction of attending Class Surveyor. OSR 1029 Sea Strainer to be renewed w/in 6 months. OSR 1067 Main Propulsion Turbo bolts - repair w/in 6mos OSR 1106 - Gas detection in D1 Cargo Hold - 40a OSR 1109 - No2 Gen Lube Oil Pump - 40a OSR 1112 - Engine 1B Jacket Water Pump - 40a OSR 1114 - Cargo Hold Secondary Bilge System frozen valves in ballast tanks - 40a. Advise Cognizant OCMI & Local US Port Class Surveyor if mission is extended.

Vessel Engine Room fire dampers for ventilation do not shut properly, adjust or repair fire dampers to the satisfaction of the attending ABS Surveyor prior to departure from port. 74 SOLAS 01 Cons II-2/16.3.1.3

WTD 4-113-1 did not operate in all open/close methods. Repair & test door in all modes to the satisfaction of the attending ABS Surveyor prior to departure from port. 74 SOLAS 97 Cons. II-1/25-9.2

Glass on remote CO2 operation control was broken for bow thruster space. Replace glass to the satisfaction of attending ABS Surveyor within 14 days. 74 SOLAS 97 Cons. I/11

Fuel Supply Solenoid Electric Junction Box cover was missing. Install cover to the satisfaction of attending ABS Surveyor within 14 days. 74 SOLAS 97 Cons. I/11

Two bulkhead penetrations in space 03-88-3 had two different kinds of fire retardant materials used. Prove compatibility & approval or replace with a approved material to the satisfaction of attending ABS Surveyor within 14 days. 74 SOLAS 01 Cons. II-2/43.1

Condensate drain line in Fan Room 01-85-3 appeared to be leaking and approx 2" of standing water were on the aft deck of space. Make repairs to the satisfaction of attending ABS Surveyor within 14 days. 74 SOLAS 97 Cons. I/11

Fire screen door in emergency escape hatch would not latch shut. Make repairs to the satisfaction of attending ABS Surveyor within 30 days. 74 SOLAS 97 Cons. I/11

Five ventilation dampeners in cargo holds would not close completely during shut down. Those ventilation openings SS28 F-Deck, SS15 E-deck, SS25 E-deck & SS51 are to remained closed until permanent repairs can be made to the satisfaction of attending ABS Surveyor no later than 07FEB2019. 74 SOLAS 01 Cons. Vessel had failure of their No. 1 main Circulation Pump Upper Bearing Housing. Repair or Replace & show proper operation to the satisfaction of attending class surveyor prior to departure from port.

crew discovered a pinhole leak at the bottom of a piping seem weld in the 20" portside raw water cooling discharge pipe. A temporary repair in the form of a make shift jubilee patch was installed to secure the leak.

WTD 6-33-1 was found to be inoperable in the closed and watertight position. Hydraulic leak was noted during operation. Repair hydraulics and prove proper operation of door. Cite: Class supplement.
