The cognizant Officer in Charge, Marine Inspection (OCMI) determines the total number of persons permitted to be carried on a vessel. In determining the total number of persons, the OCMI may consider the total weight of passengers, crew, and variable loads; stability restrictions and subdivision requirements of the vessel; the vessel's route, general arrangement, means of escape, and lifesaving equipment; minimum manning requirements; and the maximum number of passengers permitted in accordance with § 176.113 of this part. Vessel conducted a deadweight survey witnessed by the Coast Guard on June 10, 2025 at Keehi Lagoon in Honolulu, Hawaii. To comply with vessel stability criteria while awaiting MSC review to account for the additional weight added, a total of 15 Upper Deck External Passengers must be removed from the original 80 allowed. 46 CFR 176.112

Whenever a launching appliance, survival craft, rescue boat, fixed gas fire extinguishing system, machinery, fuel tank, or pressure vessel is installed aboard a vessel after completion of the initial inspection for certification of the vessel, as replacement equipment or as a new installation, the owner or managing operator shall conduct the tests and make the vessel ready for the inspections required by § 176.402(d) to the satisfaction of the cognizant OCMI. Vessel stbd generator installation is not complete. Generator is not allowed to be operated until the installation is complete and load tested to the satisfaction of the attending Marine Inspector by June 20, 2025. 46 CFR 176.702

A self priming, power driven fire pump must be installed on each vessel of more than 19.8 meters (65 feet) in length. Fire pump unable to meet pressure requirements due to pin hole leak in fire main piping located in piping section forward of starboard engine compartment bulkhead. Section of piping requires renewal. 46CFR181.300

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment. Electric submersible bilge pump hose and overboard fitting wasted and broken in aft starboard steering compartment. Requires renewal. 46CFR182.510(a)

A pre-engineered fixed gas fire extinguishing system must be in compliance with § 181.420 of this part and may only be installed in a normally unoccupied machinery space, with a gross volume of not more than 57 cubic meters (2,000 cubic feet). Port engine compartment fireboy cylinder found over pressurized. Requires maintenance from a qualified fire technician.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. The most aft port side engine room ventilation blower/fan was not operational. Provide operational aft port side engine room ventilation blower/fan operational. 46 CFR 182.100

A vessel must comply with the structural design requirements of one of the standards listed. Corrosion found on weld in way of starboard engine room escape hatch causing pinhole in overhead. Gouge and renew weld. 46 CFR 177.300 705 C

On a vessel of not more that 65 feet in length carrying more than 49 passengers, and on a vessel of more than 65 feet in length, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure on not less than 60 PSI at the pump outlet. The pump outlet must be fitted with a pressure gauge. The gauge at the pump outlet had a broken needle. Replace with operational gauge. 46 CFR 181.300(b)

Spanner wrenches must be provided where a 1.5 inch diameter fire hose is required. Spanner wrench missing. Provide spanner wrench. 46 CFR 181.310(d)

Each fire hydrant must have a valve installed to allow the fire hose to be removed while the fire main is under pressure. Valve not installed. Provide valve on fire hydrant. 46 CFR 181.310(c)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI giving consideration to provisions for a seaworthy hull. Port side hull forward suffered damage and was holed approximately 18 x 4 inches in size above the water line. Vessel allided with pier. Provide temporary repair proposal for approval. Once proposal is approved make temporary repairs and conduct watertight testing. Once temporary repair testing is completed vessel may proceed for permanent repairs on neighboring island. Make permanent repairs in accordance with NVIC 7-68 and conduct Non Destructive Testing prior to taking passengers. 46 CFR 177.100

Numerous areas throughout vessel have temporary repairs using green looking epoxy. Remove temporary repairs at drydock and make permanent welded repairs to: Port side chain locker on outboard side; port side crew storage outboard hole into bulwarks. 46 CFR 177.100

Changes to potable water storage have changed from original approved drawings. Water bladder/bags substituted for water tanks. Provide drawings/plans for water bladders/bags.

Changes to approved plans made. Starboard side bow thruster/battery/inverter installation modified. Provide drawings/plans of current installation of bow thruster/battery/inverter.

Changes to approved plans made. New water pumps installed. Provide Provide drawings/plans for newly installed water pump.

Two vertically stacked electric ovens installed. No plans submitted prior to installation for flame spread or electrical use. Provide oven installation plans, electrical plans, and technical specification for ovens.

Lifefloat cracked, torn, wasted. Provide 1 new lifefloat and receipt of purchase. Install ASAP once received.

Marking for permitted passengers at upper deck missing at entranceway to that deck. Provide second deck marking for passengers permitted.

Control wire under pilot house chafed/eaten. Remove damaged wire and replace with new section of wire.

Inverter under pilot house not in use/partially wired. Remove wiring and inverter if not in use. Provide updated drawings showing inverter removed.

Fire extinguisher at pilot station not in bracket. Provide appropriate bracket for portable fire extinguisher.

Fire pump does not have the ability to be operated locally. Fire pump must be capable of local operations at the pump.

A vessel must comply with the structural design requirements of one of the standards listed. The port shaft mount has excessive corrosion roughly 12 inches from the aft most portion of the shaft mount. Investigate cause of corrosion and if repairs are required submit a repair proposal to the satisfaction of Marine Inspector.

Pre-engineered fixed gas system must automatically shutdown equipment in space in accordance with 46 CFR 181.420. Port installed fire suppression system did not secure ventilation, MDE or generator during test.

Inspect vessel for unsafe pratices in accordance with 46 CFR 830. Powerstrips were found on second deck and bridge.

Escape hatches shall be labeled on both sides with 2" clearly legible letters "EMERGENCY EXIT, KEEP CLEAR" in accordance with 46 CFR 185.606. Port and stbd spaces aft of collision bulkhead emergency escapes were not labeled on upper side.

Lifefloat cover hydrostatic releases were expired. Renew releases in accordance with 46 CFR 176.808.

Lifefloat weak link was missing tag. Renew weak link in accordance with 46 CFR 176.808.

Vessel was missing 1 10-B:C portable extinguisher. Provide extingusher in accordance with 46 CFR 176.810.
