Vessel is authorized a one time transit from Martha's Vineyard to Woods Hole with passengers and cargo. A crew member shall maintain a watch in the space that was damaged during the entire voyage. Vessel is authorized a one time transit from Woods Hole to Fair Haven to complete permanent repairs without passengers or cargo. Repairs are to be made to the satisfaction of an attending Marine Inspector. Prior to transiting to Fair Haven all cracks shall be drill stopped and a temporary repair shall be made to the damaged area to make it water tight.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs, or a defect is discovered which affects the safety of the vessel. Following vessel allision with the Woods Hole Oceanographic Institute dock, observed an inset in the hull/rub rail in the vicinity of the port bow, aft of the mooring access side shell opening, aligned approximately in the center of the scupper openings. Inset in the rub rail is approximately 2 feet long and 2-3 inches deep, with slight deflection of the hull plating beneath the chine.
