The local operation of the non-powered hand pump fails to actuate the door. Doors affected are WTD #1, #3, #6, #7, and #8. Provide closures IAW regulation to the attending USCG MI. 46CFR 170.270.c.

Lifesaving appliances and arrangements ...are in satisfactory condition, and remain fit for the service. The life ring on the main passenger deck, STBD-FWD, is no longer fit for service. Provide IAW regulation. Corrected on the spot. 46CFR 199.45

In order to be accepted by the Commandant, any spark ignition engine fitted to an approved rescue boat must...MSC Circ. 980, be stopped by the helmsman's emergency release switch as attached. The clip on the rescue boat is broken and not able to be fastened to helmsman. Provide IAW regulation. Corrected on the spot. 46CFR 160.156-7

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. Vessel's port engine #2 cylinder exhaust port was leaking jacket water. Jacket water was discovered on the #2 piston head. Identify the cause of the leak and provide the attending marine inspector with a technician report certifying that proper repairs have been completed and the engine operates as designed.46CFR70.20

Port side ventilation on the freight deck is not operating. Repair and re-test ventilation shutdown.

Vessel reported an exhaust leak on the starboard main engine turbo charger.

Vessel has large pieces missing from the ends of each of the 4 blades on the port propeller. Vessel is to proceed to Thames Shipyard located in New London, CT for permanent repairs. No cargo and no passengers (no commercial operations) are authorized until repair or replacement of the port prop is completed to the satisfaction of the cognizant OCMI. 46CFR71.55-5

VITAL SYSTEMS THAT ARE AUTOMATICALLY CONTROLLED OR REMOTELY CONTROLLED MUST BE PROVIDED WITH AN EFFECTIVE PRIMARY CONTROL SYSTEM. THE PRIMARY CLUTCH CONTROL SYSTEM (STBD MDE) FAILED TO "CLUTCH-IN" UPON DEPARTURE FROM MARTHA'S VINEYARD (BOTH FROM BRIDGE CONTROL AND ECR CONTROL). VESSEL CLUTCHED IN EMERGENCY MODE.

Vessel suffered damage to the side shell, deck, and structural members between frames 47-49 stbd side. If during, internal structural examination, damage or deterioration to the hull plating or structural members is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs.Provide repair proposal for the damaged area of the side shell and deck between frames 47-49 and make temporary repairs to the satisfaction of the attending USCG MI.

Vessel suffered damage to the rub rail and deck between frames 47-49 stbd side. If during, internal structural examination, damage or deterioration to the hull plating or structural members is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. Temporary repairs made, conduct and log weekly inspections of repaired area for 4 weeks and monthly thereafter until permanently repaired. Make permanent repairs to the satisfaction of the attending USCG MI prior to next credit drydock scheduled for Jan 2025.

While backing into the slip in Martha's Vineyard the vessel struck the dock with the starboard quarter causing a dent and crack in the vessel's hull. Vessel is authorized a one time transit from Martha's Vineyard to Woods Hole with passengers and cargo. A crew member shall maintain a watch in the space that was damaged during the entire voyage. Vessel is authorized a one time transit from Woods Hole to Fair Haven to complete permanent repairs without passengers or cargo. Repairs are to be made to the satisfaction of an attending Marine Inspector. Prior to transiting to Fair Haven all cracks shall be drill stopped and a temporary repair shall be made to the damaged area to make it water tight.

Where pressure-indicating gages are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of graduations, and the orientation of the display will be considered. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. The installed gauges on the 03 air receivers are out of this range for the 275 PSI relief devices. Provide gauges IAW with regulation. 46CFR 54.15-5 (f)

Operating generators must provide a continuous and uninterrupted source of power for the ship’s service load under normal operational conditions. Vessel experienced a loss of of the #2 generator. Troubleshoot and conduct repairs to the satisfaction of the attending USCG Marine Inspector prior to returning the #2 generator in service.

Each cable installation must meet Sections 25.11 of IEEE 45-2002. "Metal supports should be designed to secure cables without damage to the armor or the insulation. The supports should be arranged that the cable would be supported for a length of at least 13 mm. cable retention devices should be installed not less than every 610 mm on vertical runs and not less than every 2.5 m on horizontal runs." Provide metallic supports in the identified location along the aft main space bulkhead, vertical installation.46 CFR 111.60-5 IEEE 45-2002

Valves must be so designed for the maximum pressure to which they may subjected ... The fire service valve of station 23 shows signs of salt water leak while under pressure. Provide installation of leak free operation.46 CFR 56.20-9 (c)

Doors in "A" Class bulkheads shall meet the following requirements ... The vestibule leading in to the Engine Control Room is identified as a Class A boundary IAW approved fire plan. The door is A-60 self-closing. Provide adjustment to ensure door functions as designed.46 CFR 72.05-25 (b)

The vessel owner or operator must ensure that the following are specified: ...Lock or otherwise prevent access to unattended spaces. The security plan must be adhered to at all times. Designated areas are to remain secured from unauthorized individuals. The security door midship, main deck (vehicle load) on port side would not close as designed. Provide secure installation.33 CFR 104.265

Approved fire extinguishing systems must be installed in auxiliary spaces containing internal combustion engines. Required fixed fire extinguishing system 34 second time delay for the Bow Thruster space failed testing discharging at 31 seconds. Replace time delay.

Each class 3 door must be Be designed, constructed, tested, and marked in accordance with ASTM F 1196. Prove proper operation of the #3 watertight door. Door would not open/close/open on hydraulic accumulator and once power was regained would not open.

A stability test of a vessel may be dispensed with if the Coast Guard determines that an accurate estimate of the vessel’s lightweight characteristics can be made and that locating the precise position of the vessel’s vertical center of gravity is not necessary to ensure that the vessel has adequate stability in all probable loading onditions. Vessel installed a partial monorail system in the engine room. Submit weight change calculations to the Marine Safety Center to verify stability and lightweight characteristics.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs, or a defect is discovered which affects the safety of the vessel. Following vessel allision with the Woods Hole Oceanographic Institute dock, observed an inset in the hull/rub rail in the vicinity of the port bow, aft of the mooring access side shell opening, aligned approximately in the center of the scupper openings. Inset in the rub rail is approximately 2 feet long and 2-3 inches deep, with slight deflection of the hull plating beneath the chine. At next dry dock, effect repairs to the satisfaction of the attending Marine Inspector. 46CFR71.40-1(a)

Vital systems that are automatically or remotely controlled must be provided with an effective primary control system. Port MDE ECM/PLC inoperable causing MDE to be inoperable. Repair to satisfaction of Marine Inspector.

Tag out fuel oil purifier & provide MSC approval of as built drawings prior to placing into service.

Provide final MSC approval of the DVTP/PSTPs for the switchboard.

Re-submit an ammended as built drawing (plan number 3233-173-03) for F.O. purifier electrical system.

#1 generator keel cooler replacement

Provide USCG MSC approved DVTP and PSTPs for the main switchboard and test to the satisfaction of the attending USCG Marine Inspector. Testing extended to 31Jan2021.

Verify and revise master vessel PSTPs to reflect actual testing performed and submit for review and approval.

EMERGENCY ESCAPE HATCHES NOT OPERATIONAL (TO HARD TO OPEN FROM BELOW) AND BLOCKED WITH SALT SPREADING EQUIPMENT BT MACH SPACE AND VOID #2. REPAIR OR REPLACE, REMOVE EQUIPMENT HANGERS THAT ENCOURAGE STORAGE ABOVE HATCHES, PAINT AND STENCIL HATCHES EMERGENCY ESCAPE KEEP CLEAR AT ALL TIMES

The vessel owner/operator must obtain an approved NTVRP Approval Letter by 14 Dec 2019.

STBD engine not working on all cylinders. Make repairs to STBD engine prior to carrying passengers to the satisfaction of Marine Inspector.

Vessel's #5 sliding watertight door located adjacent to the engine room is not operating as designed. Both local and remote controls for the electro-hydraulic pump system do not work. Vessel is still able to open and close the door with the hand-pump. The #5 WTD shall remain closed at all times until it has been inspected by a CG Marine Inspector.

Repair strobe lights for fire alarm system in the engine room, auxiliary machinery room, and emergency generator space. Stobe lights are inop.

Identify and repair cause of steering failure alarm sounding in pilot house while operating the steering system using local controls in the steering compartment.

No audible alarm for loss of power on the steering system was sounding in the pilot house. Repair.

Troubleshoot and repair watertight door #7. Power handle actuator only works intermittently to open the door. Ie..will not open the door when moved to the fully open position and has to be exercised back and forth several times in order to get the door to open.

Replace the expired EPIRB hydrostatic release device

Repair the inoperable lower masthead light

Properly repair the forward fire screen doors on both the port and starboard sides

Repair the inoperable PA speaker on the 02 level aft starboard side

Repair the inoperable GA Bell on the 02 level aft starboard side

Update and submit to USCG MSC the A/C 1 line diagram to accurately reflect the wiring for the two new capstans and the crew galley hood.

Update and submit to USCG MSC the fire detection plans to reflect the relocated relays and breakers in the pilothouse panel.

Provide evidence on what employees are considered HazMat Employees, who has completed HazMat training IAW 49CFR176.13 and 172.704, and provide records of such training.

Vessel reported having problems with the propulsion controls located in the wheelhouse. Master received error fault informing master to proceed with caution as the controls for the controllable pitch propeller (CPP) are not operating as designed. Vessel's master shall notify a CG Marine Inspector prior to getting underway from Vineyard Haven.

Vessel's port controllable pitch propeller is not operating as designed. Port propeller became stuck in 100% forward pitch while transiting from Vineyard Haven to Woods Hole, MA. Also, port controllable pitch propeller lags in changing pitch from going from zero pitch to astern pitch. Vessel needs to diagnose problem and repair to the satisfaction of a Coast Guard Marine Inspector.

Vessel operations restricted with passengers onboard to propulsion controls in MODE 1 only and within weather conditions at the Captains discretion. All other propulsion control modes shall be disabled while passengers and cargo are on board.
