Fire-detection systems. Except as provided in paragraph (a)(8) of this section, each towing vessel must have a fire-detection system installed to detect engine room fires. The fire detection panel was found to be inoperable. Repair or install a fire detection system in accordance with 46 CFR 142.330. Per your request the vessel may operate with an additional trained crew member to assume a fire watch in the engine room at all times while the vessel is operating. The additional crew member shall be equipped with a radio in case of an emergency. An alternate crew shall be provided when the vessel is away from the dock for a period exceeding twelve (12) hours in any twenty-four (24) hour period. The vessel may operate under these terms until 01 JULY 2026 or until the fire detection panel is back restored to full operational status, whichever occurs first.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: (5) High bilge levels. Bilge aft alarm inop. Prior to rectification, crew shall conduct hourly rounds of aft bilge during vessel operations.

A fixed fire pump must be capable of: (1) a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm)). Vessel's fire main had a gauge installed, but was inop.

Verification of compliance with the construction and arrangement design standards of this part [Part 144] must be performed before the new installation is performed. A new generator was installed on the vessel, which was not a replacement-in-kind, prior to completion of the verification of compliance. To show compliance and IAW 46 CFR 144.145 provide a copy of the verified plan to Sector UMR, OCMI and make vessel available for examination of new installation.

The owner or managing operator must ensure the fire-detection system is maintained in accordance with the manufacturer's design manual. The horn/strobe indicator in the engine room does not strobe when the heat detection probes are activated.

The following systems must be equipped with gauges at the machinery location: Auxiliary generator engine RPM. No RPM gauge was present for the generator.

Vessel required to be fitted with navigation lights to display when aground and not under command. Vessel not fitted with lights to display when around (nor "not under command" (Rule 27)) which are used to meet display requirements for meeting aground display requirements).

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Inspection found 160B semi-portable fire extinguisher is not permanently secured. Prove to the attending Coast Guard marine inspector that the extinguisher is properly secured.

A vessel of 12 meters or more in length shall be provided with a whistle. Whistle inoperable at time of inspection. Whistle required to be made operational.

Work vest storage container not required to be marked with the words "Work Vest". Storage container not marked with the words "Work Vest". Required storage container to be properly marked, or, work vest removed from container.

Currently installed emergency lights failed to energize automatically as required. Install emergency lighting for all internal crew working areas.

Portable, battery powered, light not installed at the access to the engine room. Install portable light at engine room access.

Fire alarm horn/strobe (high decibel audio & visual strobe light) located at operating station is not able to be silenced or extinguished and has potential to significantly distract master's ability to operate vessel in a safe manner during an emergency. Remove horn/strobe from operating station.

Fire detection system failed to actuate during testing of engine room heat detector. Repair system to ensure proper function from sensor in engine room.

Vessel's two lifebuoys were fitted with lifelines attached. Vessel required to have at least one lifebuoy without a lifeline attached. Ensure one lifebuoy does not have a lifeline attached.

Records not available for lifesaving equipment testing and maintenance (as required by 46 CFR 199.190) to include date, name of person, and any receipts. Make records available for exam.

Magnetic compass required to be illuminated. Wiring was not connected to illuminate compass. Connect wiring to properly illuminate compass.

Records not available for Safety Orientation Training (46 CFR 140.410), Navigation Safety Training (46 CFR 140.645), & Emergency Drills & Instruction for abandon vessel and response to threats to the environment (46 CFR 140.420(a)). Provide records with required entries to include training/drill date(s), description of training/drills, names of training/drill providers, names of training/drill participants. (Safety Orientation Training record required to include signature of individuals providing training.)

Fall overboard prevention procedures did not include use of personal protective equipment (if any), line handling safety, and safety moving to/from the vessel. Update procedures to include these items.
