The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Four boiler tubes were found damaged restricting vessel to operate at 50 RPM (14 knots). 74 SOLAS (14) II-1/26.1 Code 17 A

Ships constructed on or after 1 January 2010 shall be provided with means of embarkation on and disembarkation from ships for use in port and in port related operations, such as gangways and accommodation ladders, in accordance with paragraph 2, unless the Administration deems that compliance with a particular provision is unreasonable or impractical. A container was dropped and damaged the vessels starboard side accommodation ladder. The accommodation ladder is not reparable and must be completely replaced. 74 SOLAS (14) II-1/3.9.1 Code 50A
