All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. To address the rudder issue, the crew replaced the two missing bolts and used an impact gun to torque the nuts. As a temporary measure, since welding underwater was not feasible, an air hammer chisel was used to bend and distort the bolt threads behind the nut to prevent it from falling off if it were to become loose. Make permanent repairs to the satisfaction of attending CG Marine Inspectors prior to 01JUN2026. 46CFR182.100

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. While getting underway from the pier the vessel experienced a failure of one of the main diesel engines. It was determined silicone had fallen into the fuel tank during dry dock, resulting in the clogging of the fuel filter and the engine failure. Fuel filters were cleaned and engine resumed operation.
