BEFORE THE SHIP LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE ALL LIFE-SAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. PSCO OBSERVED 30 OF THE REQUIRED 32 IMMERSION SUITS HAD A FAILURE WHICH ALLOWED THE INSULATING LAYER TO SEPARATE FROM THE SUIT WHICH POSES A THREAT TO THE CREW IN THE EVENT OF EMERGENCY.

Access door to forward BOSN storage port-side (FR 135P) intrusion alarm unit was duct taped in the closed position falsely indicating a closed access door.
