PSCO identified a modification made to the port side anchor windlass in which an access hole has been enlarged to better allow the crew to reach the windlass break. the crew states that the modification was made at sea. The crew also states that ABS has not been on board to approve this modification.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO identified an oxygen tank in the oxygen tank locker placed on the deck and tied with rope to the bulkhead.
