Prior to carrying passengers conduct security, fire and abandon ship drill's and verify required number of fire suits are on-board and satisfactory. All to the satisfaction of attending CG Marine Inspectors.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the serious marine incident. Alcohol test strips were not on board the vessel. Provide test strips to the marine inspector. 46 CFR 4.06-15(a)

46 CFR 199.180 Prior to carrying passengers complete security, fire and abandon ship drill's, to include lowering, operation, and recovery of the port lifeboat.

46 CFR 61.40-10 Periodic Safety Test Procedure (PSTP) document must be in a step-by-step or checkoff list format and must specify equipment status, apparatus necesary to perform the tests, safety precautions, safety controls and alarm set points, the procedure to be followed, and the expected test results. The following PSTPs from test from draft PSTP Rev C, Elliot Bay job # 13001.02, dated 2/19/21 must be updated: 1) Main Fuel Tank(s) Low Level Alarms: because the test was not conducted IAW the PSTP, this will need to be revised to articulate the acceptable method of testing performed on 21 April – lowering the set point to a level just below that which is in the tank at the time of testing. When editing the PSTP for MSC review/approval, add another step to requiring the set points be reset to 25%. 2) Accommodation Sprinkler Flow Alarms: this test was not included in the PSTP. Add this test procedure to the PSTP revision being submitted to MSC for approval. 3) Main Propulsion Back-up Power Failure: once proper testing procedures and outcome is received from Kongsberg is received, update this test procedure. 4) Fired Thermal Fluid Heater Testing: when we conducted the testing on 26 April, it was determined that the lower temperature set point of 140 degrees was not able to be achieved due to ambient temps and the lack of demand for heat. This procedure will need to be amended to account for a “summer” and “winter” mode, and will need to account for the use of gator clips when jumping the analog switch.

46 CFR 62.35-50 (table) The status of the main propulsion installations are required to be indicated at remote locations. When testing the Fire Relay Shutdowns, the main propulsion engines shutdown as required/designed, however, the PMC IMACS monitoring system in the Engine Control Room showed the engines were still running. Provide proof to a Coast Guard Marine Inspector that the status of the main propulsion installations is accurately indicated when the Fire Relay Shutdowns are activated.

Remove and replace all plastic trash cans onboard

Properly connect fire hose in fire station 15 to hydrant.

Provide schematic diagram of the engine room fixed fire system at storage bottle location.

Update posted Emergency Instructions for Method of donning lifejackets to include all styles of lifejackets onboard or change all lifejackets to one style to match current posted Instructions.

Provide SMC that list vessel type as per DOC

Provide proof of weekly testing of fire detection system as per the SMS or update SMS.

Update Lifesaving plan to include the total number of children’s life jackets at the storage locations

Multiple fire doors throughout vessel have mechanical holdbacks. Remove holdbacks.

Unapproved drill press installed in a hazardous location (paint locker). Remove unapproved drill press.

Route of escape from bow thruster room, engine room and fwd machinery space not marked with escape route or exit signs. Install illuminatated exit signs.

Multiple fire stations throughout vessel had loose or missing nut for valve wheels. Ensure all hose station valve wheels are secure.

Single line diagram for installed fire fighting sysytem for engine room controls was not part of instructions as per cite. Provide schematic for instructions.

First aid kits on car deck and food prep room contained expired items. Remove and replace expired items.

Ring buoy on 3rd deck stbd side has a clip attaching line to light. Remove clip and splice line to body oof ring bouy.

Press in fwd machinery space is not secured in place. Secure press in place.

Remove plastic trash can in galeey.

Emergency lighting shall be tested weekly IAW cite.Provide documentation that lighting is being tested.

Emergency lighting for mustering and abandonment must be tested at each abandon ship drill. Lighting was not energized during abandon ship drill. Provide documentation that lighting is being tested during abandon ship drills.

Multiple Periodic Safety Test Procedures (PTSP's) tested were incorrect, out of set point parameters or had incomplete information to complete. Systems must be tested as per PSTP's by operator and evaluated for validity. Any procedures incorrect or invalid must be amended and resubmitted to Marine Safety Center for approval. Once final approval has been received, PTSP's must be demonstrated and proved to attending Coast Guard Marine Inspector.
