While acting as a standby vessel, vessels are required to have on board 02 searchlights. 01 searchlight on the pilothouse was inoperable, leaving only 01 searchlights available on the vessel. Must be fixed prior to vessel operating at a standby vessel.

Where rigid nonmetallic material (plastic) is permitted for use in piping systems by this section, the following restrictions apply: Penetrations of required watertight decks and bulkheads by any rigid plastic pipe are prohibited unless the following requirements are met: Each penetration must be accompanied using an acceptable metallic through deck or through bulkhead fitting that is welded or otherwise attached to the bulkhead or deck by an acceptable method and one or more metallic shutoff valves must be installed adjacent to the fitting. MI observed leaking from the shaft seal raw water cooling watertight bulkhead penetrations from the generator room to the main engine room. Rectify to the satisfaction of the USCG. 46CFR182.720(d)(1)

On a vessel that has a load line, the amidships marks required by paragraph (c)(2) of this section must be those required by the International Convention on Load Lines, 1966. MI observed that the vessel's deck line was not painted in contrasting color to the hull. 46CFR185.602(e)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for the safety under normal and emergency conditions. MI discovered a broken breaker on the bridge utilized for the VHF #3 radio. Repair/replace to the satisfaction of the USCG. 46CFR183.200(a)

Working space must be provided around all main distribution panels and switchboards of at least 610 millimeters (24 inches) in front of the switchboard, and at least 455 millimeters (18 inches) behind the switchboard. MI witnessed multiple packages and equipment stored behind the switchboard panel prohibiting the required working space. Rectify to the satisfaction of the USCG. 46CFR183.330 (e)

Paint and flammable liquid lockers must be constructed of steel or equivalent material, or wholly lined with steel or equivalent material. The vessel is currently outfitted with an aluminum paint locker. Rectify deficiency to the satisfaction of the USCG. 46CFR177.405(d)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall have the vessel ready for inspection including watertight closures. Watertight doors in engine room and in generator room were not watertight and required adjusting. 46 CFR 176.802 46 CFR 175.400

Machinery and equipment installed on each vessel must be suitable for its operation and for the purpose intended to afford adequate protection from machinery failure and personnel injury. Exhaust bracket was missing two bolts and three nuts. 46 CFR 182.100

The draft or loading mark mus be painted in a contrasting color to the sideshell paint. The draft marks were not painted. 46 CFR 185.605

Vessel has not completed its annual load line survey. Vessel is not to operate beyond the boundry line until the Load Line survey has been conducted.

Anchor line shackle was missing a tool (i.e. pliers) for removing a coder pin fastened through the shackle bolt to retain the nut.

Flush hatch covers were not securely attached with suitable metalic clips or shackles and chain.

Vessel did not have an annual inspection (due NLT 25JUN18) within the alloted 3-month timeframe following the anniversary date (25MAR2015) of the COI.
