Survival craft must be stowed in accordance with the requirements of 199.130 of this chapter. Life float line not attached and cover secured to not allow free floatation.

Fuel piping, non-metallic flexible hosing is acceptable if it is used in lengths of not more than 30inches. Flexible fuel hoses were approximately 7ft long.

Essential systems or equipment must be regularly tested and examined. Temperature gauge on both generators not functioning, and port engine alarms displaying starboard alarms.

Fixed fire pump must be capable of delivering water at pitot-tube pressure of 344kPa. Fire pump gauge too large of scale to accurately read pressure.

(a) Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. No protection such as lifelines, wire rope, chains or fixed rails installed.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. relief valve was set at 150 psi with the air receivers having an MAWP of 135 psi for the air compressors.

The following systems must be equipped with gauges at the machinery location: Main engine lubricating oil pressure and main engine rpm. Main engines did not have RPM gauge.

Electrical systems, general. Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. pilot house battery just has wire connected to battery post without a terminal lug.

Life buoy must be marked in block capital letters with the name of the vessel. Name of the vessel is not in block capital letters but is written.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms defined in 46 cfr subpart 30.10, into the machinery space. Port generator has a slight fuel leak. vessel will tag out generator and still meet the existing vessel requirements with one generator until parts arrive.

46CFR140.435 First aid equipment. Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions.

Each pyrotechnic distress signal carried to meet this section must be stowed in A portable watertight container carried at the operating station. Portable watertight containers for pyrotechnic distress signals must be of a bright color and must be clearly marked in legible contrasting letters at least 0.5 inches high with “DISTRESS SIGNALS” in accordance with 46 CFR 141.375(e). The vessel was using a non watertight container that was unmarked to store the distress signals. Provide a suitable, orange, watertight container and label as noted above to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

Towing vessels must be equipped with a fathometer in accordance with 46 CFR 140.725(a). The fathometer on the vessel was stolen and has not been replaced. The Master did record the missing equipment in the TVR. Provide a new fathometer to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

Each life float on a towing vessel must be fitted with a lifeline, pendants, a painter, and floating electric water light approved under approval series 161.010 in accordance with 46 CFR 141.325(c). This includes a sea painter in accordance with 160.010-4(j). There was no sea painter attached to the life float and the retro-reflective tape was no longer serviceable. Provide a suitable sea painter of at least 100 feet in length and secure around the body of the life float and affix retro-reflective tape to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of 46 CFR 199.190 and in accordance with 46 CFR 141.235. During a review of the lifesaving inspection and maintenance record, there were several items marked not applicable and other found in a condition not identified in the inspection report. Conduct all inspections and maintenance of all lifesaving appliances and make accurate entries into the TVR that meet the criteria in 46 CFR 141.235(b)(1) to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022

On a towing vessel, essential systems or equipment must be regularly tested and examined at intervals as described in 46 CFR 143.245(b) and recorded in the TVR. during a review of the Record of examination and tests, it was determined some of the tests were not being carried out in the intervals prescribed by the regulations. Conduct all tests in the table in 46 CFR 143(b) and record the same in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

Each towing vessel must be equipped with emergency lighting in all internal crew working and living areas and must provide sufficient illumination under emergency conditions to facilitate egress in accordance with 46 CFR 143.410(b). The emergency lighting in the engine room was missing and there was no means of lighted egress in the event of an emergency from the engine room. Install emergency lighting to illuminate the exit of the engine room and demonstrate proper operation of the emergency lighting system for the engine room to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

Each towing vessel must be equipped with at least two portable, battery-powered lights. One must be located in the pilothouse and the other at the access to the engine room in accordance with 46 CFR 143.410(c). There was no portable light near the access of the engine room. Provide one portable light in the vicinity of the exit of the engine room to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

Each remote valve control for the fuel systems for the main engine and generators on a towing vessel, must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it in accordance with 46 CFR 143.260(e). The remote fuel shut off markings faded and not clearly visible to the crew. Properly label the emergency fuel shut off valves, indicating the tank that will be shut off and the way to operate the valve markings must be in clearly legible letters of at least one inch in height to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022.

The type and quantity of chemicals that are required to operate the device are provided by the manufacturer in accordance with 33 CFR 159.57(a)(5). The vessel was not adhering to the manufacturer’s instructions for operating the system as they were using chemicals that were not listed in the manufacturers instructions and were not following the instructions for operation of the Marine Sanitation Device. Provide the proper chemicals and ensure the Marine Sanitation device is operated in accordance with the manufacturer’s instructions to the satisfaction of an attending Coast Guard Marine Inspector prior to August 16, 2022

Each towing vessel must have a fire-detection system installed to detect engine room fires and the system draws power from two sources. Switchover from the primary source to the secondary source may be either manual or automatic in accordance with 46 CFR 142.330(a)(5). During a test of the Fire Detection System, the system was still displaying a fault for the emergency battery back up. Determine the cause of the emergency battery fault and provide servicing report from a technician certified in fire alarm systems prior to the satisfaction of an attending Coast Guard Marine Inspector prior to August 3, 2021.

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, smoke detectors and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months in accordance with 46 CFR 142.240. Properly document all the tests and inspections for the fire detection system and all smoke detectors in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to August 3, 2021.

Each towing vessel must have means be provided for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces in accordance with 46 CFR 144.605. The vents in the funnel stack did not have a means to to close off the natural air ventilation to the space. Provide a means of closing the vents in the event of an engine room fire to the satisfaction of an attending Coast Guard Marine Inspector prior to August 5, 2021

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The port side running light was inoperable when tested at the beginning of the inspection. Prior to completion of the inspection, a member of the crew replaced the bulb in the running light and the proper operation of all navigation lights were confirmed prior to completion of the inspection. CORRECTED ON THE SPOT.

Each life float used as a survival craft on a towing vessel must be fitted with an electric water light approved under approval series 161.010. in accordance with 46 CFR 141.325(c). The floating water light was burning continuously while in the inverted position. A membor of the crew made adjustments to the light configuration and the battery installation. Prior to completion of the inspection the floating water light was tested and found to be fully operational. CORRECTED ON THE SPOT.

Users must always maintain their Automatic Identification System in an effective operating condition and ensure the accuracy of all broadcasted dynamic, static and voyage information in accordance with 33 CFR 164.46(d). This vessel currently is transmitting a previous call sign and MMSI number. Update the broadcast information on the AIS to include the correct MMSI Number and Call Sign to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The master of each towing vessel must keep an accurate list of crew members and their assigned positions and responsibilities on board the vessel in accordance with 46 CFR 140.400(a). Personnel records do not have assigned positions. Properly document the personnel records in the TVR to include the names of each crew member and their assigned positions on board in the Towing Vessel Record (TVR) to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The master of each towing vessel must record the date and time when a navigation watch stander, including the master, officer in charge of a navigational watch, and lookout assumes a watch and is relieved of a watch in the Towing Vessel Record (TVR) in accordance with 46 CFR 140.400(c). Properly document the personnel records in the TVR to record the date and time when each watch stander assumes and is relieved of the watch to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Safety orientations and other crew training must be documented in the TVR in accordance with 46 CFR 140.410(d). The vessel did not have any records of a Safety Orientation for any crew members who recently embarked on the vessel. Properly document the safety orientation for all crew members in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The records in the TVR must be kept available for review by the Coast Guard upon request. Records, unless required to be maintained for a longer period by statute or other federal regulation, must be retained for at least 1 year after the date of the latest entry. The vessel was unable to produce records when requested. Provide Safety Orientation, Personnel Records, Examinations and Tests, Navigational Assessment, Towing an Terminal Gear Maintenance and Inspection, Fire Fighting Annual Instruction and Fire Fighting Inspection and Maintenance Records to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The owner, master, or operator of each towing vessel of less than 1,600 GT shall ensure that the Operation of all internal vessel control communications and vessel-control alarms are operating properly before the vessel embarks on a voyage of more than 24 hours or when each new master or operator assumes command in accordance with 33 CFR 164.80(a)(3). Ensure all communication equipment is tested, operating properly and is recorded in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway, including The operational status of pilothouse instrumentation and controls, to include alarms, communication systems, variation and deviation errors of the compass, and any known nonconformities or deficiencies in accordance with 46 CFR 140.635(a)(5). Ensure Navigational Assessment of all alarms and communication systems is completed and a record of the tests is documented in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Inspections and tests of lifesaving equipment must be recorded in the TVR including weekly visual inspections of the survival craft and a test of the general alarm system in accordance with 46 CFR 141.235. Ensure all visual inspections and tests of all lifesaving equipment and the general alarm are completed and a record of the tests/inspections is documented in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

All Receipts and other records documenting the inspections and tests of all lifesaving equipment must be retained for at least 1 year and made available upon request in accordance with 46 CFR 141.235(b) and 46 CFR 199.190 Provide the records of lifesaving appliances tests and inspections for the last one year to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months in accordance with 46 CFR 142.240. Properly document all the tests and inspections for all fire fighting appliances in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The owner, master, or operator of each vessel towing alongside or pushing ahead shall ensure that the face wires, spring lines, and push gear are frequently inspected and remain in serviceable condition In accordance with 33 CFR 164.76. The results of the towing gear inspection must be captured in the TVR in accordance with 46 CFR 140.820(a). Conduct an inspection of all towing and terminal gear and capture the results of that inspection in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The master of a towing vessel must ensure that each crew member participates in fire-fighting drills and receives instruction at least once each month in accordance with 46 CFR 142.245. Ensure all fire fighting drills and instructions is carried out with all crew members and is recorded in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced in accordance with 46 CFR 143.245 and tests required in Table 143.245(b). A results of these tests must be recorded in the TVR. Conduct all tests of essential systems and equipment and a record of the tests and examinations is documented in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Fuel systems for towing vessel main engines and generators must have a documented maintenance plan to ensure proper operation of the system in accordance with 46 CFR 143.255. Provide a documented maintenance plan for all fuel systems for the main engines and generators to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The owner or managing operator must implement a health and safety plan in accordance with 46 CFR 140.500. The plan must identify and mitigate the health and safety hazards outlined in 46 CFR 140.510 and be compiled in one single plan but was missing procedures for noise (140.510(1(a)(8)) and embarkation and disembarkation of the vessel (140.510(1(a)(10)). Provide the completed health and safety plan to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Each towing vessel must have a fire-detection system installed to detect engine room fires and must identify the zone or zones of origin of the fire in accordance with 46 CFR 142.330. Identify and label all zones protected by the installed Fire Detection System so the person on watch can readily identify the location of the fire to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Each lifebuoy on a towing vessel must be fitted with retro reflective material that meets IMO Resolution A.658(16) in accordance with 46 CFR 141.360. Properly affix retro reflective material as per the IMO Resolution to the Life Ring Buoy on the bridge wing to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Survival craft must be marked with the vessel name in accordance with 46 CFR 199.176(b)(1). Properly mark the name of the vessel on the rigid life float in block capital letters to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Towing vessels must be equipped with a Search light, controllable from the vessel's operating station in accordance with 46 CFR 140.725(b). The vessel has two searchlights but one is not operable. Make repairs and demonstrate proper operation of the searchlight to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

Each towing vessel must be fitted with a general alarm Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light in accordance with 46 CFR 143.235(b)(3). The vessel has an installed light but is blocked by an overhead frame and is not sufficient to warn a person in the machinery space. Remove or reposition the flashing red light so that it is visible in the entire space to the satisfaction of an attending Coast Guard Marine Inspector prior to June 12, 2020.

The type and quantity of chemicals that are required to operate the device are provided by the manufacturer in accordance with 33 CFR 159.57(a)(5). The vessel was not adhering to the manufacturers instructions for operating the system as they were using the wrong additive (RID-X) and the wrong chlorinating tablets (pool grade). The instruction manual stated that the additive to be used is SDE 340 and only waste water treatment chlorinating tablets are to be used. Provide the proper chemicals and ensure the Marine Sanitation device is operated in accordance with the manufacturers instructions to the satisfaction of an attending Coast Guard Marine Inspector no later than June 5, 2020.
