Training manual and Form E require immersion suits worn with life jackets. Immersion suit present did not require a life jacket per SOLAS approval to be worn with one yet life jacket was donned during abandon ship drill.

Materiel deficiencies noted during the inspection dated 31May2022 and approximately 114 overdue maintenance items present in NS5, show concern of the effectiveness of implemented SMS. An additional external verification is required to determine root cause of the continued lack of maintenance, the results of which to be submitted to ABS and USCG, ABS to verify the adequacy of the proposed corrective action.

Jacket Water Shutdowns are designed by manufacture to shut down at 95C. #1 and #4 did not shut down with temperature in excess of 110C. #3 shut down at 101C. #2 could not be tested due to repairs on engine.
