The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards. The vessel experienced an equipment failure while at berth in the Houston Ship Channel.
Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored.

Sufficient foam concentrate shall be supplied to ensure at least 30 min of foam generation.
The Fixed Deck Foam Firefighting System manual section 4.6 says a volume of 8100L is required to meet the 30 min requirement.
Current tank level per manual sounding is 148 cm. Per sounding table, this corresponds to approximately 7450-7500L.

Cargo samples stowage space shall be cell-divided, and samples which react with each other dangerously shall not be stowed close to each other.
PSCOs observed multiple cargo samples not cell-divided, causing bottle-on-bottle, including in some cases glass-on-glass contact. Additionally, multiple samples have no identifying labels, preventing proper segregation.
