All tank vessels, the construction or conversion of which is started on or after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. The hull and deckhouses shall be of steel or iron construction except that the pilothouse and decks over quarters may be constructed of wood. Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Vessel reported damage 75 Ft forward of the stern above the water line after a collision with a fishing vessel. Vessel is to enact permanent repairs to the satisfaction of the Coast Guard.

Each barge equipped with an anchor, to comply with 33 CFR 155.230(b)(1) must be fitted with an operable anchoring system that includes a cable or chain, and a winch or windlass. All components of the system must be in general conformity with the standards of a recognized Class society. Vessel anchor, chains, and cabling detached from vessel IVO Galveston Fairway Anchorage. Vessel shall complete repairs prior to cargo operations.

No flammable or combustible liquids may be stowed in any accommodation, control, or service space (other than a paint locker). No more than 19 liters (five gallons) of flammable liquids may be stowed in any machinery space. The flammable liquids must be in containers of 3.8 liters (one gallon) or less. No more than 208 liters (55 gallons) of combustible liquids may be stowed in any machinery space. An aggregate of more than 7.6 liters (two gallons) of flammable or combustible liquids stowed outside of an accommodation, control, or service, space (other than a paint locker) or outside of a machinery space must be stowed in a paint locker that is marked with a warning sign indicating flammable or combustible liquid storage. Excessive amounts of 5 gallon buckets of oil and multiple 55 gallon drums were being stored on deck.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Numerous issues were found with the life buoys on board. Covers were over the buoy, reflective tape was not in satisfactory condition and the home port was not labeled

A pressure gage shall be installed for each pump discharge, and it shall be located at a point visible with respect to the pump controls. The 02 forward cargo pumps did not have the capability of monitoring the pipe pressure at the pump control location.

The discharge containment required by §§ 154.530, 155.310, and 155.320 of this chapter, as applicable, is in place and periodically drained to provide the required capacity; Vessel had standing water in the containment underneath all transfer manifolds. Vessel was using the containment area as misc storage.

The requirements of § 58.10-5 (a), (c), and (d) apply to diesel engine installations. (Engine design. All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. MI identified a noticeable leak off of generator in machinery room.

The frame or support of each size III, IV, and V fire extinguisher required by table 34.50-10(a) must be welded or otherwise permanently attached to a bulkhead or deck. The 160 b semi portable fire extinguisher bracket was not welded or otherwise permanently attached

Each cargo hose must pass a hydrostatic pressure test at ambient temperature of at least one and a half times its specified maximum working pressure but not more than two-fifths its bursting pressure. Hoses on board were only tested to 150 PSI not the minimum 225 working pressure required for cargo hoses by 46 CFR 32.50-30. Hoses must be tested prior to use.

No stowage position or muster and embarkation station for a survival craft on a tank vessel may be located on or above a cargo tank, slop tank, or other tank containing explosives or hazardous liquids. Both liferafts on board were stowed on the cargo tank deck. Move to approved location on barge prior to next credit drydock.

Each fire extinguisher shall be marked with a number and the location where stowed shall be marked in corresponding numbers in at least 1/2 inch figures. Multiple fire extinguishers were found not marked appropriately.

Vessel required to have flamescreens installed on diesel fuel tanks. 04 diesel fuel tanks missing 30" x 30" flamescreens. Install prescribed flame screens in accordance with 46CFR30.10-25.

The vessel is required to have a mechanical means of adequately closing each drain and scupper in the enclosed deck area. The port for the mechanical plug was wasted and needs to be renewed. Renew port and ensure mechanical means of closure works.

Vessel cargo hoses are to be marked with the OIL SERVICE / MAWP prior to use in accordance with 33CFR154.500. Vessel's cargo hoses were not marked with OIL SERVICE / MAWP. Mark cargo hoses.

The 240V panel in the accommodation space has improper ground detection. The ground detection on that panel is an amp to ground meter.

THE OFFICER IN CHARGE, MARINE INSPECTION MUST BE NOTIFIED BEFORE ALTERATIONS OR MODIFICATIONS THAT DEVIATE FROM APPROVED PLANS. LOCK OUT/TAG OUT BLOCK HEATERS UNTIL APPROVAL FOR INSTALLATION OF EQUIPMENT AND ASSOCIATED WIRING IS RECEIVED. Rectify def prior to operation of the equipment.

Lifejackets need to be marked with name of vessel in accordance with 46 CFR 199.70(b)(3).

Immersion suits need to be marked with name of person assigned or name of vessel in accordance with 46 CFR 199.70(c)(3).
