PSCO discovered 03 of 06 main engine cylinders steadily leaking jacket water. 01 cylinder is leaking more significantly than the rest at a rate of 25-30 drops every 10 seconds.

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. The design shall have regard to the working conditions to which it will be subjected and enviornmental conditions on board. PSCO discovered the hydraulic system for the hatch covers had leaked oil into the BOSUN STORES. The oil covered the I-beams above the system, the hoses, and outer casing, as well as the containment area.

0618-Stowage of rescue boat-Rescue boast shall be stowed in a state of continuous readiness for launching in not more that 5 minutes, and if the inflated type in a fully inflated condition at all times. PSCO's noted that the inflatable rescue boat was not maintained at full air pressure. Crew added air and there didn't appear to be any visible leakage. SOLAS(14) III/14 10c
