The ship shall be divided by thermal and structural boundaries to contian a fire to its space of origin. The PSCO observed ne ducting that passed through class A bulkheads in the several accomodation spaces to include laundry and passageway. The space between the duct and the penetration fitting was sealed with duct tape and not a suitable non-combustible material. 74 SOLAS (04 Cons) II-2/9.1

Resolution: Class report attesting that incombustible sealing compound was placedin the space around the ducting inside the bulkhead fittings.

After any survey of a ship has been completed, no change shall be made to structural components covered by the survey without the santion of the administration. The PSCO observed new ventilation duct penetrating class A bulkheads within accomodation spaces, and the penetrations were sealed with duct tape and not a suitable non-combustible material. 74 SOLAS (04 Cons.) II-2/9.1

After any survey of the ship is completed, no change shall be made in the equipment covered by the survey without the sanction of the administration. The PSCO observed 4 mooring hawsers with nylon strap spliced/woven into the strands at verious points to reduce chaffing. 74 SOLAS (14 Cons) I/11.

In meeting these requirements the company should ensure that inspections are conducted at appropriate intervals and appropriate corrective action is taken. The ships crew has not conducted inspections on hawsers every 3 months as required by SMS and accurate record of wear was not kept. 74 SOLAS (14 Cons) IX/5 ISM Code 10.2
