74 SOLAS (14) IX/3.1, ISM CODE CLAUSE 1.4.2 Every company should develop implement and maintain an SMS which includes instructions and procedures to ensure protection of the environment. Ship did not follow Valve seals Procedure VOP-04-08 which requires bilge system... after test or inspection, [to be] fitted with a numbered, easily breakable seal in compliance with the controlled valve procedure. Vessel found with Engine Room main emergency bilge suction (valve WSO12F) and both engine room general service #1 and #2 pump valves found with seals connected to zip ties creating an ineffective valve seal arrangement capable of being bypassed without breaking the numbered seals. SMS audit required to lift detention deficiency.

Ship did not properly follow Controlled valve procedure 4.0 which requires "secure valves identified to be kept closed using yellow prolock device." Yellow prolock device on overboard valves for the following systems did not lock properly as they require human engagement to tighten and be secured. Following overboard valves were able to be opened by hand without key required by SMS: 1. shipside soot drain eductor overboard. 2. Sewage overboard valve. 3. Grey water overboard valve. 4. OWS overboard valve. SMS audit required to lift detention deficiency.
