2510 - Safety and Environmental Policy - The company should identify equipment the sudden operational failure of which may result in hazardous situations and should provide for specific measures aimed at promoting the reliability of such equipment. Crew stated they use personal protective multi-gas meters during cargo operations and bump check them prior to use. Vessel does not have valid and correct span gas for calibration and bump checks. Current span gas expired FEB 2019. Must be corrected prior to cargo operation to the satisfaction of Coast Guard Marine Inspector. - SOLAS 74 (2014 Con.) Ch IX/2.3 & ISM Code (2008 ED.) Part A/10.3

1899 - Other (tankers) - Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter diameter and is at least 25.4 millimeters long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. The permanent stud is missing, having been broken off during prior operations on the starboard forward vapor line. - 33 CFR 39.2001(j)

Resolution: The vessels crew affixed a bolt/stud into the appropriate position. CG Marine Inspector verified the correction.
