Fuel shutoff valves were not clearly marked. Label fuel shutoff valves to properly identify location of valve.

Ring buoy perimeter grabline is degregated and fraying. Repair or replace to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 180.70(b)

Conduct and log man overboard, fire, and abandon ship drills with newly reported crewmembers and every 3 months. Provide Coast Guard Marine Inspector objective evidence (log book) of drills conducted. 46 CFR 185.420& 46 CFR 185.520& 46 CFR 185.524

he master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. During drill crew-member working over the water had not donned a life jacket. Add this step to vessel's emergency instructions. Vessel's Master also determined the best side to bring victim to meet EMT would most likely be the "Canalside" side, due to possible quicker response by onshore emergency responders. Add verbiage to emergency instructions stating that (depending on circumstances at the time) bringing a victim to meet onshore first responders may be best done on the Canalside side of the river.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel has defective gangway due to fractured welds. Gangway must be satisfactorily repaired or replaced to the approval of an attending Marine Inspector. A person must be stationed in the immediate proximity of this gangway to monitor the safe arrival and departure of passengers.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit contents expired. Replace required contents of first aid kit with unexpired items.

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions posted did not follow the actual actions of the ship's crew in case of a Man Overboard (MOB). MOB instruction made note of crew member entering water to assist MOB which is unsat. Make posted emergency instructions as to the actual emergency practices of the vessel and drill procedures of the crew.

Each floating waterlight must be approved in accordance with §161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Waterlight battery was not changed out ye4arly as per manufacturer. Change waterlight battery and mark with date of install.

The stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel.

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. MMC's were not onboard for examination. Produce credentials and medical certs for examination.
