Resolution: Ultimate cause deemed to be water contamination of fuel oil for vessel's generators. Affected tanks drained, refilled, and tested for water. All generators tested satisfactorily under load.

Vessel's hatch maintenance plan was not available for inspection as it is electronic and computer could not be opened. SOLAS(20) XII/7.2

Bulk carriers shall comply with the maintenance requirements provided in the "standard owners' inspection and maintenance of bulk carrier hatch covers", adopted by the organization by resolution MSC 169(79). PSCO noted the following areas on non-compliance: a. Bearing surfaces and drainage channels were full of debris and lacked cleanliness. b. Hatch cover drains and their non-return valves were missing fire caps. c. PSCO noted the ship's maintenance plan and record of maintenance component replacement was not found to be in compliance with IMO resolution MSC 169(79).
