Resolution: Ultimate cause deemed to be water contamination of fuel oil for vessel's generators. Affected tanks drained, refilled, and tested for water. All generators tested satisfactorily under load.

Bulk carriers shall comply with the maintenance requirements provided in Reg II-1/3-1 and standards for owners, inspection and maintenance of bulk carrier hatch covers, adopted by the Organization by Res MSC.169(79). Vessel's hatch maintenance plan was not available for inspection as it is electronic and computer could not be opened. SOLAS(20) XII/7.2

Ships constructed on or after 1 July 2002 shall be fitted with navigational systems and equipment which will fulfill the requirements prescribed in paragraphs 2.1 to 2.9; Each vessel required to be fitted with an Automatic Radar Plotting Aid (ARPA) under § 164.38 of this part must be fitted with a device to indicate speed and distance of the vessel either through the water or over the ground. Vessel reported an inoperative speed log. Provide documentation attesting to repair of speed log prior to next US port after sailing foreign.
