The vessel's captain reported the discharge of the wet chemical fire suppression system (Gaylord system) protecting the griddle and deep-fat fryer in the galley. The cause of the discharge is unknown; however, no evidence of a fire or manual activation was observed by the attending Marine Inspectors. The system is currently discharged and non-operational. Corrective Action Required: -The griddle and deep-fat fryer shall remain locked out and tagged out (LOTO) of service. -The Gaylord fire suppression system must be restored, serviced, and certified to a fully operational condition by a qualified marine fire protection technician. -The system's operational status and servicing certification must be verified and cleared by an Marine Inspector prior to the removal of the LOTO and the resumption of cooking operations on the affected appliances. 46 CFR 76.01-5(a)

All doors, shall be of a type which are capable of release from the control station and from a position at the door. Fire Screen Door(FSD) #6 handle prevented door from properly operating. FSDs on car-deck stairwell failed to close during fire-drill. Repair FSDs to satisfaction of attending Marine Inspector.

On vessels required to have a final emergency power source, the emergency lighting must be arranged so that they can be energized from the final emergency power source. Emergency lighting on upper deck port side crew accommodation areas were not working under emergency power. Identify and fix emergency lighting so that lights operate as intended under emergency power.

Each machinery space must be ventilated to ensure that, when machinery or boilers are operating at full power in all weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery and for the safety, efficiency, and comfort of the crew. MI observed engine room supply fan SF-13 (Port Side) fails to start and could not be tested for a shut down. Do not use supply fan, until the unit has been repaired to original and verified by attending MI. To be corrected by next credit drydock. 46 CFR 58.01-45

The list of required plans is general in character, but includes all plans in 79.65 which normally show construction and safety features coming under the cognizance of the coast guard. Fire plan revised during COI inspection. Updated all fire control plan in all location as outdated by the fire control plan. 46CFR71.65

The general construction of the vessel shall be such as to maintenance fire hazards insofar as is reasonable and practicable. Upper deck fan room and upper deck forward port service trunk have excessive storage or combustible materials. These spaces do not have fire suppression installed. Remove all combustible materials in excess and/or install suppression in spaces. 46CFR72.03-5

The inspections and tests must ensure that the equipment and associated structure are in satisfactory operating conditions and fit for the service for which they are intended. PSTP FRS-1: Zone 1 & 2 supply fan 14 & 13 do not stop running upon activation of zone 1 & 2 fire relay vent switches. RST-3 (Engine room FM 200 release) & RST-4 (EOS FM 200 release): Fire dampeners in engine room space do not close upon activation of the test switches on the FM 200 pressure switches. 46CFR70.20-1/46CFR61.01-1(b)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this sub chapter. Vessel reported a high temperature reading on the port main propulsion engine, main bearing. Port engine went temporarily off line, causing a reduction of propulsion. Identify cause and repair prior to sailing 31MAR2025@ 0700. 46CFR70.20-1/46CFR58.05-1

Electric installations on vessels must ensure Maintenance of services necessary for safety under normal and emergency conditions. Vessel experienced power failure and could not recover normal or emergency power to navigational equipment. Demonstrate proper operation of vessel electrical system to satisfaction of Marine Inspector. 46 CFR 111.0-1

Doors in bulkheads required to be Class A–60, A–30, or A–15 shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A–15 bulkhead. Fire door 4-FD5 was found to have a missing door handle, leaving 3 holes in the fire boundary.

Operating generators must provide a continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Any vessel speed change or throttle movement must not cause a ship's service load power interruption. Vessel Chief Engineer reported a loss of the ships port service generator while underway on 21JAN. The generator functionality was maintained as the starboard generator quickly assumed the load. Vessel is to ensure that both generators are available while transiting. Ensure USCG attendance, provide technician report and monitor for 30 days to ensure no further incidents occur.

Fire doors must be installed IAW fire control plan. Fire doors are required to be self closing per the fire control plan. Location of the doors: door for the galley, door between scullery and crew mess, and door between crew mess and passageway.

Fire dampers shall be desigend to close against the anticipated draft in the duct. Unable to verify proper operation of Fire Damper #38 located in the car deck.

Means shall be provided to close of all ventilators. The local ventillation shutdown with-in the galley was inoperable.

Bulkheads shall be designed IAW its designated fire boundary. The follow doors were unable to close shut to maintain the assigned boundary: (1) galley door, (2) laundry door next to the galley, (3) door between galley and scullery, (4) women's restroom door on the upper deck.

Signage - In each crew berthing areas and washing areas information on the procedures and resources to report crimes, including sexual assault and sexual harassment, must be adequately posted. These procedures and resources must include at a minimum the following information: 1. Company policies regarding sexual assault, sexual harassment, and drug and alcohol usage. 2. Telephone numbers, websites and email addresses for reporting allegations of sexual assault and sexual harassment to the Coast Guard. 3. Company procedures to report violations of company policy and how to access resources. 4. Resources provided by outside organizations such as sexual assault hotlines and counseling. Provide a copy of informational bulletins addressing policies and procedures per the Safety Management System to the satisfaction of the attending Marine Inspector. 46 U.S.C. § 11101

Master [name withheld] - The owner of a vessel subject to inspection under section 3301 shall ensure such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to an established list of crew, by position, allowed to access and use the master key. This list must be maintained onboard the vessel and included within vessel’s safety management system. Additionally, a log book must be maintained with records of access and use of the mater key and shall include the following information at a minimum; date and time of access, room or location accessed, and name and rank of crewmember using the master key. This log book is required to be included in the safety management system. It may be in electronic format and shall be located in a centralized location that is readily accessible to law enforcement personnel. Provide a plan to incorporate a master key control system and recordkeeping procedures onboard the vessel to the satisfaction of the attending Marine Inspector. 46 U.S.C. § 3106

Safety Management System - With respect to sexual harassment and sexual assault, procedures for and annual training requirements for all responsible persons and vessels to which this chapter applies on: Prevention; bystander intervention; reporting; response; and investigation. Provide a plan to incorporate the sexual assault and sexual harassment requirements into the Safety Management System to the satisfaction of the attending Marine Inspector. 46 USC 3203 & 46 USC 3205; Reference: CVC-WI-004(3) Flag interpretations of the ISM Code.

An audible and a visible alarm must activate in the pilothouse upon — (1) Failure of the electric power to the control system of any steering gear; (2) Failure of that power to the power unit of any steering gear; or (3) Occurrence of a low oil level in any oil reservoir of a hydraulic, power-operated steering-gear system. Vessel Master reported inoperable port steering gear alarm panel. Restore panel and test to the satisfaction of a CG Marine Inspector. 46 CFR 58.25-25(d)

Vessel Master and Chief Engineer reported inoperable port steering gear alarm panel in the EOS. Restore panel and test to the satisfaction of a CG Marine Inspector. 46 CFR 58.25-25(e)

The hydrants in interior locations may have wye connections for 1 1/2-inch hose. In these cases, the hose must be 75 feet (22.86 meters) in length, and only one hose will be required at each fire station; however, if every interior space can be reached by a 50-foot hose then 50-foot hoses may be installed at each interior fire hydrant. Identified two fire hoses on board that were not compatible to be threaded to other hoses or atatched to fire station due to different hose part number and hose design. Remove fire hoses from vessel and ensure only compataible fire hoses are on board vessel..

Operating generators must provide a continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Any vessel speed change or throttle movement must not cause a ship's service load power interruption. Vessel has reported multiple un-commanded port and starboard generator shutdowns spanning previous 30 days. Vessel is required to provide technician report and findings of what is causing the shutdowns and to provide proof of proper operation of generators, bow thruster and associated systems, prior to carrying passengers.

Correct and submit the periodic safety test procedures to the USCG Marine Safety Center due to multiple tests having different procedure or results from the test not following the step-by-step requirement of 46 CFR 61.40-10. After it has been reviewed demonstrate to the satisfaction of attending Coast Guard Marine Inspector.

In accordance with 46 CFR 113.43, a steering failure alarm must be installed in the pilothouse of vessels over 1600GT. The alarm system must be separate from, and independent of, eachsteering-gear control system, except for input received from the steering wheel shaft. Complete installation within 30 days to the satisfaction of attending CG Marine Inspector.

Re-submitt vessel security plan to MSC with an ammendment to reflect the new construction details of the berthing as employee access areas. When submitting use the vessel case number 690391.

The Galley equipment below the exhaust hood wiring has been altered from original design. Submit plans to MSC showing details of the controller used to automatically secure power to the fryer, oven, hot plate, and toaster, in an emergency. Compliance to the motor circuit requirements can be shown either on the One-Line distribution drawing or in a separate motor controller drawing.

The elevator means of communication is not connected and is required by ASME A17.1. Means of two-way conversation between the elevator and watchman is required and must be continuously monitored to take action for emergencies. Observe the safety of the elevator every half hour, until repairs are made. Complete installation within 30 days to the satisfaction of attending CG Marine Inspector.
